Employee and another employee were closing a garage door and the cable broke causing one of the rollers to come out hitting the employee in the head requiring 2 staples.

Employee was opening the inspection door on the sampler system in the load out building and it struck employee's left index finger and cut it. Resulted in 5 sutures and employee returned back to work next scheduled shift.

Employee was reaching for ticket and when she tried to sit back down the chair rolled away causing her to fall on desk and floor. Started missing work on 10/19/2015.

MSHA inspector observed coal at #1 and #4 silos smoking and issued a K-order #8321634. A citation for not reporting the incident was issued, citation #8321634, for failure to report. No actual fire was observed. This is being reported to prevent further action against the operator.

EE was walking and stepped out of scale house turning her left ankle. Was taken to Hazard ARH and diagnosed with a severe sprained ankle. Returned to work.EE worked til July 1st with no restrictions doctor excused her from work because of swelling in foot.

Chute was plugged with large blocks of coal at crusher. A slate bar was being utilized with a sledge hammer to break the material free. Sledge hammer glanced off of bar being used and struck victim in right forearm resulting in cracked ulna bone.

OPERATOR WAS OPERATING A 980C CAT LOADER CLEANING UP DUMP PAD AND BACK INTO COAL TRUCK. LOADER OPERATOR WENT TO HOSPITAL WITH COMPLAINTS OF BACK PAIN. XRAYS WERE NEGATIVE. HE RETURNED TO WORK AND CONTINUED TO WORK UNTIL 3-25-02. HE CALLED AND SAID DR. PUT HIM OFF FROM WORK.

EE AND COWORKERS WERE WORKING ON A BREAKER BOX THAT CONTROLLED THE CRUSHER (TYPO TIPPLE). THE BREAKER EXPLODED CAUSING BURNS TO 80-90% OF EE'S BODY. 26% TO ONE COWORKER AND BURNS TO THE HANDS OF ANOTHER COWORKER. ALL THREE WERE TRANSPORTED TO HOSPITAL VIA AMBULANCE.

EE WAS STANDING 6-10' AWAY FROM THE BREAKER EXPLOSION WHERE TWO COWORKERS WERE WORKING. HE RECEIVED BURNS TO HIS HANDS WHILE TRYING TO EXTINGUISH THE FIRE ON THE COWORKERS. HE WAS TRANSPORTED TO HOSPITAL WHERE HE WAS TREATED AND RELEASED 2 DAYS LATER.
