EE STATED HE WAS BACKING THE DOZER FROM A COAL PILE THAT MEASURED 3' HIGH AND AS THE REAR OF THE DOZER NEARED THE EDGE OF THE SMALL PILE, THE DOZER TIPPED, CAUSING THE REAR OF THE DOZER TO STR IKE THE CONCRETE PAD. EE STATED, WHEN THE DOZER STRUCK THE CONCRETE PAD HE WAS JARRED IN THE DOZER'S SEAT.

ON 01-14-00 EMPLOYEE REPORTED THAT HE FELL AT A FLOOR DRAIN AND STATED THAT HE WAS NOT INJURED. HE CONTINUED TO WORK REGULAR UNTIL 02-18-00 WHEN HE INFORMED MANAGEMENT THAT HE WAS EXPERIENCING KNEE AND BACK PAIN FROM THE 01-14-00 INCIDENT. WENT TO DOCTOR 03-02-00 DID NOT TAKE OFF WEEK AS DOCTOR SUGGESTED. THIS EMPLOYEE HAD PREVIOUS NON WORK RELATED INJURY AND COMPANY WAITING ON POS
