EE was cutting a coupling. He accidentally stabbed himself in the leg. It required 4 stitches. He only missed a few hours work.

The foreman discovered a rock fall on the belt line during the pre-shift. It was on the #1 belt flight at the #10 cross cut on the #2 entry. The fall was approx 35' long, 25' wide and 8' thick. This fall was at an intersection. The foreman reported to the safety director at approx 6:10 am.

EE STEPPED ON A CAP BOARD THAT HAD A NAIL IN IT. THE NAIL WAS BIG ENOUGH TO PENETRATE BOOT AND FOOT. EE DID NOT MISS ANY WORK, BUT DID GO TO DOCTOR'S OFFICE TO GET A TETANUS SHOT.

EE WAS HANGING CAR CABLE AND GRABBED A BARE LEAD. HE CONTINUED WORKING FOR MOST OF HIS SHIFT BUT THEN DECIDED TO GO TO HOSPITAL. TESTS WERE RUN, NO PROBLEMS FOUND. HE WENT BACK TO WORK THE NEX T DAY.

EE WAS USING A BAR TO MOVE STRAPS, WHEN A STRAP SLIPPED CAUSING BAR TO HIT HIM ON LEG. THE IMPACT WAS TO THE SHIN AND CAUSED IMMEDIATE BRUISING AND PUT A DENT INTO LEG. WENT TO LOCAL HOSPITAL, X-RAYS SHOWED NO BREAK. THIS OCCURED ON THURSDAY. EE TOOK OFF REST OF WEEK AND RETURNED TO WORK MONDAY 7-22-02 WITH NO RESTRICTIONS.

A ROOF FALL WAS DISCOVERED AT THE 327 CROSSCUT OF THE NO. 2 MAINS IN THE BELT ENTRY. THE FALL WAS FOUND ON 7-3-02 AT 6:00 PM WHILE THE MINE SUPT WAS CHECKING THE MINE DURING A IDLE SHAFT. THE FALL MEASURED 6 FT THICK 30 FT LONG AND 30 FT WIDE.

THE ROOF FALL OCCURRED SOMETIME BETWEEN 1400 ON 10-13 -01 AND 0440 ON 10-15-01 WHEN IT WAS DISCOVERED. THE ROOF FALL BEGINS INBY SPAD NO 3+65 AND EXTENDS APPROX 150' ON THE BELT CONVEYOR.
