Technician was attempting to repair a steering problem on a wheel loader. He replaced steering pilot valve, started engine, raised bucket and attempted to steer machine. When it would not steer to the right, he dismounted machine without shutting off engine. While attempting to remove steering shaft, the steering activated and steered to the right crushing the victim.

OPERATING 980 FRONT END LOADER, WHILE TAKING A BUCKET OF GRAVEL DOWN A HILL, EE LOST CONTROL OF THE END LOADER. EE JUMPED FROM THE LOADER BEFORE THE FRONT END LOADER WENT INTO A DITCH.

Employee was moving a wooden pallet which had a metal band attached to it. As he threw the pallet the metal band cut his left thumb.

OPERATOR WAS REMOVING SERVICE DOOR ON LOADER WHEN HE LOST HIS FOOTING & FELL 5' TO THE GROUND, WHERE DIRECT FORCE TRAUMA RESULTED IN BOTH WRISTS BEING FRACTURED. WORK AREA HAD BEEN WET DOWN FOR DUST CONTROL PURPOSES & WAS MUDDY AS A RESULT OF SUCH ACTION.
