Tightening the brake on a rairoad car and the release handle on the brake wheel struck his wrist. he began missing work on the 5/13/04. it has been determined by the medical information from the doctor, that this was caused by the 12/15/03.

THIS IS AN ALLEGED INCIDENT. EMPLOYEE ALLEGES HE SUFFERED A HERNIA ON THE NIGHT IN QUESTION WHILE MAKING REPAIRS TO A COAL CHUTE. HE DID NOT REPORT THE INCIDENT TO ANYONE UNTIL THE FOLLOWING D AY WHEN BEING REPRIMANDED FOR POOR WORK.

EMPLOYEE WAS CLEANING THE AREA UNDER THE TRUCK SCALES WHEN A TRUCK PULLED ONTO THE SCALES HE WASSTARTLED. HE SUDDENLY RAISED UP AND STRUCK HIS BACK AGAINST A BEAM. HE CONTINUED TO WORK UNTIL 4 -13-00.
