The employee and co-workers were in the No.8 entry at x-cut No.47 on the 3 South Mains preparing to remove a permanent stopping. The injured employee was in the process of walking outby to turn off a water valve, when the roof collapsed resulting in a fatality. MSHA was notified. The injured employee was pronounced dead by the coroner on at 10:48pm on 3/21/2022.

Employee and Co-worker were removing rock off continuous miner. As EE was removing the rock, the rock slipped pinning EE's finger against another rock which resulted in a laceration to a finger on EE's left hand.

Employee was rock dusting the face of the entry by hand. The employee was in a bent over position, as they moved backwards they came to an area where the roof was higher. When the employee stood upright they felt pain in lower back.

EE was installing a roof bolt and when EE raised the drill pot, to attach the wrench to the bolt head, the bolt caught and bent, mashing EE's pinky finger to the frame of the drill head.

Employee was removing a guard from the transfer jack off of a ram car, and when they lifted the guard they strained their lower back. Employee finished the shift and went to a local clinic for medical treatment.

Employee was replacing the guard over the tail shaft on a feeder. The guard slipped and caught the employee's left hand resulting in a simple fracture on EE's index finger. The employee did not go to the hospital until 01/22/2021 and was released the same day.

Employee sought medical attention on this date after months of reoccurring pain in lower back. An MRI was obtained and the results shows that Employee has degenerative disc disease from L3-L4 to L5-S1. Under the advise of the physician, employee began missing work on 2/21/2017 due to the condition.

The area was originally supported with 36" resin bolts and timbers. All persons were moved outby until the escape way was re-routed. No persons were affected.

As worker was helping align a feeder during a set up, went around the feeder to look at the alignment. As EE walked around the feeder, slipped and fell striking chest on a crib block that caused a contusion to chest. EE began missing work on 5/11/2016 when EE had the contusion drained by a physician.

On this date, employee reported that he had been suffering back pain. He sought medical advice and began missing work on 12/7/2015 to seek treatment. Results of a MRI shows anterolisthesis and degenerative changes to his L4-L5 disc. He remains off work to continue treatment.

No injuries occurred due to this event.

The fall area was previously supported with 5' Hy-tech bolts and cribs. The fall area was cleaned up and resupported. There were no injuries or equipment affected with this fall nor was ventilation affected.

The roof bolter was backed out into the #3 entry at crosscut #12 and employee was on inby side of bolter changing a coupling of a piece of drill steel when a loaded ram car came out of the heading and pinned employee between the car and roof bolter resulting in multiple injuries to his pelvis.

Employee was assisting another worker install a roof bolt. When the mast was lowered, it caught his right hand between the drill pot and mast base causing multiple fractures to the hand and the loss of the ends of his index and middle fingers which were removed during later surgery.
