Plant Mechanic stepped down from a Ford F-250 plant truck, and turned left ankle when EE touched the floor with their foot. There were no obstructions on the floor at the time of the incident.

Employee was walking down a flight of stairs on the secondary plant when his right ankle rolled over when stepping from the bottom step to the landing, resulting in a sprained ankle. Steps were dry and free of debris. No lost work days, restricted duty for two working days.

Employee was freeing a lodged flat rock from a head box with a sledge hammer, when a small piece of rock deflected off the top of his safety glasses and hit him on the forehead above his safety glasses, causing a one inch cut above his right eye.

Employee stepped on soft ground while walking across Plant lot and twisted left knee. At first Dr. said it was a sprain. Employee was later sent to Dr. who said employee had some torn cartilage. Later the Dr. decided to do surgery on 8-25-04. There was no lost time until 8-25-04.

9/11/01 EE WAS CHANGING A SPEED REDUCER ON THE #4 CONVEYOR. AS EE LIFTED THE SPEED REDUCER HE FELT A PAIN IN BACK WAS REPORTED AS 1ST AID CASE. NO MED TREATMENT WAS REQUIRED. 3/4/02 SAW DR FOR FIRST TIME FOR BACK PAIN RESULT OF 11/09/01 IN CIDENT. RET'D TO DR ON 4/26/02 & WAS UT OFF WORKTHRU 5/24/02
