EE was driving a 35 ton backdump. He was going uphill with a full load. The load shifted back and the transmission shifted down at the same time allowing more torque to pull wheels. This allowed front wheels to lift off ground into standing position. When load came off truck came down.

EE's backdump was being worked on so the supervisor told EE to help clean up in shop. EE was sweeping the floor when his elbow accidently tipped over a container of tire seal and it splashed in his eyes. He then went and cleaned his eyes out with eye wash.

ROCK GOT CAUGHT IN DOOR OF THE #25 BIN EE WAS USING SHOVEL TO KNOCK ROCK LOOSE. ROCK ROLLED DOWN THE SHOVEL AND HIT HIM ON THE FOREARM.

EMPLOYEE WAS WAITING AT TOP OF HILL TO GO DOWN INTO THE HOLE, BACKDUMP DIED, EE HAVING NO CONTROL OVER THE EQUPMENT, EE'S BACKDUMP RAN INTO THE SIDE OF ANOTHER BACKDUMP, WENT OVER THE HIGHWALL THEN FALLING APPROX 35 FT INTO THE HOLE.

EE "CAUGHT A RIDE" WITH OTHER EE TO RETURN TO REGULAR JOB ACTIVITY WHEN BACKDUMP'S BRAKES FAILED, CAUSING THEM TO GO FAST DOWN RAMP. EE APPARENTLY GRABBED STEERING WHEEL TO AVOID BEING THROWN AROUND IN THE VEHICLE, CAUSING THE BACKDUMP TO TO TURN OVER ON ITS SIDE.

ANOTHER EE CAUGHT A RIDE TO RETURN TO HIS BACKDUMP WHEN THIS EE'S BRAKES FAILED, CAUSING THEM TO GO FAST DOWN THE RAMP. OTHER EE APPARENTLY GRABBED THE STEERING WHEEL TO AVOID BEING THROWN ARO UND THE VEHICLE, CAUSING THE BACKDUMP TO TURN OVER ON ITS SIDE.
