Ash Grove Aggregates Inc · Caught in, under or between a moving and a stationary object

Employee was moving a steel plate to close a set of doors and smashed is left hand between the corners of the steel. He has a small puncture wound that required cleaning our and a butterfly closure. No stitches required.

Wind blew door shut of lube truck on right 4th digit, latching finger in door. Finger lacerations and slight fracture of digit.

Ash Grove Aggregates Inc · Struck by falling object

Employee was cleaning around the plant with the skid steer loader. A rock came out and hit the bucket, bounced into the cab and hit him in the face. Rock broke his safety glasses which in turn, cut his eyebrow, bridge of nose and jerked his neck and shoulder. No stitches were needed.

Employee was cleaning up around the plant with a Skidsteer and was bitten by a spider. He was treated with antibiotics. The infection got worse and he was ordered to stay home and keep still.
