EE was performing maintenance on secondary crusher, when he dropped a wrench. He stepped off the platform on the secondary to the conveyor to retrieve the wrench. In the process, he turned his right ankle.
Employee was reaching to dislodge a rock that was caught and another rock fell on his hand. Rock broke little finger of right hand.
LOADING ANFO ON PICKUP. PULLED PICKUP FORWARD AND LEFT PICKUP RUNNING IN NEUTRAL AND WENT TO SHUT DOOR OF ANFO TRAILER. PICKUP ROLLED BACK AND PINNED EMPLOYEE BETWEEN PICKUP AND TRAILER.
