Employee was climbing onto the dozer and EE slipped and fell spraining EE's left ankle.
Employees were taking a pipe apart and working on a winch when the mechanic came around the corner to communicate to the employee inside the pilot house of the dredge, when he stepped into the pipe access.
Grinding on a flange when a piece of steel got into the employee's right eye. He was wearing his shield and glasses. Steel was not embedded; used antibiotic eye drops as a percaution.
Heavy winds blew out the glass window of the dredge that employee was working on and caused lacerations to forhead.
Two workers were moving a galvanized pipe across the shop floor. One worker was bent over in an awkward position when he went to push the pipe. He pushed on the pipe and felt a slight pull in his abdomen. EE suffered a pulled muscle in his abdomen and was seen by a doctor and is on light duty.
On July 30, 2009, employee was changing a return roller on a conveyor when the roller in his left hand fell away, pulling his left arm and shoulder. The injury was reported on August 3, 2009 but employee did not seek medical treatment until August 19th at which time he was placed on restricted duty, making this a reportable injury.
This accident just became reportable. Employee was stepping down on a work platform. His left leg gave way as he was stepping down, twisting his left knee. He remained full duty and did not want to see a doctor, however, it continued to bother him so on 2/17/09 he went to the clinic and it was discovered that he had a meniscus tear.
Employee was clearing brick sand chute and accidently ran his thumb over a sharp piece of steel. Thumb was cut and required two stitches
This is not an employee. He is a customer, He was dusting his tailgate off because it was loaded with sand. His tailgate shut on his right ring finger.
Employee had a reaction and was taken to the clinic for a shot. The substance causing the reaction is unknown. He was not bitten by an insect nor were there any chemicals. I phoned MSHA and they advised that this is a reportable accident because he had a shot for treatment and it occurred on mine property.
Employee was pulling on a cable. He was in a bent-over position and strained his lower left back. Employee is on modified duty for a short time.
Employee was beating frozen sand off of equipment his hand slipped, and his right index finger hit the gate which cut his finger. He required stitches in his finger. This has not affected his job but the doctor said to keep it dry.
Was on the dredge puting pump together and was rolling the face plate on the deck. It weighed 200 hundred pounds and he strained his lower back.
Employee was climbing on the ladder of a front end loader, slipped and hit his chin.He had to have stitches in his chin.No lost time, no modified duty.
Employee claims that running a loader has resulted in a rotator cuff tear. This would be repetitive motion injury.
Employee was working on booster pump and was using a wrench. The wrench slipped and hit employees index finger on the left hand. He required stitches.
Employee was fusing plastic pipe. He got his hand caught between the pipe and mashed his hand. There are no broken bones, however, employee's hand was extremely sore.
CABIN DOOR OF WORK BOAT SLAMMED SHUT & SMASHED EE'S THUMB - THE WIND CAUGHT THE DOOR. RIGHT THUMB.
EE SLIPPED ON ICE AND AT THE SAME TIME THE TRUCK DOOR SLAMMED ON HIS RIGHT KNEE.
EE LOST HIS BALANCE ON THE LOADER. HE STARTED TO FALL BACKWARDS AND INSTEAD JUMPED OFF. HE HUNT HIS LEFT KNEE.
EE WAS CLIMBING DOWN FROM A CRANE. HE SLIPPED & CAUGHT THE HANDRAIL, BUT HE FELL OFF ANYWAY HOLDING ON TO THE HANDRAIL. HE HAS A PULLED RIGHT SHOULDER. HE THOUGHT IT WOULD GET BETTER SO HE DID NOT REPORT IT UNTIL 8/14/03.
EE MISSED A STEP OR LOST BALANCE AND FELL OFF STEP CAUSING A RIGHT SHOULDER INJURY. THE DOSTOR THOUGHT IT WAS JUST A BRUISE, HOWEVER IT CONTINUED TO BOTHER HIM SO THEY DID AN INJECTION WHICH M AKES THIS REPORTABLE.
WE WERE JUST NOTIFIED ABOUNT THIS ACCIDENT, EMPLOYEE DID NOT TURN IT IN WITHIN THE 10 DAYS. EMPLOYEE WAS PULLING ON A WRENCH WHEN THE WRENCH BROKE LOOSE & JERKED HIS ARM.
EE JERKED ON A WRENCH AND FELT A PULL IN HIS RIGHT SHOULDER, HOWEVER, HE THOUGHT IT WAS ONLY HIS RIGHT SHOULDER, HOWEVER HE THOUGHT IT WAS ONLY MINOR. IT IS STILL BOTHERING HIM SO HE REPORTED IT ON 5-23-02. HE MIGHT EVENTUALLY HAVE SURGERY.
EE WAS WASHING OUT UNDER THE ROCK PLANT. WHEN HE EXITED HE RAN INTO A STEEL BEAM WITH HIS MOUTH AND BROKE HIS PARTIAL. ALSO HE CUT HIS MOUTH. HE DID NOT NOTIFY US UNTIL DECEMBER.
EMPLOYEE OPERATES A LOADER. HIS LEFT SHOULDER HAS BEEN BOTHERING HIM FOR SOMETIME. HE WENT TO THE DOCTOR AND HE ADVISED THAT IT WAS WORK RELATED & INVOLVED REPETIVE MOTION.
EE WAS INSTALLING DRAIN TUBE & WAS PUTTING ROCK OVER IT WHEN HIS FEET SLIPPED OUT FROM UNDER HI8M & HE FELL ON HIS REAR.
EE CAUGHT LEG BETWEEN TWO TIMERS. THEY WERE MOVING TIMBERS. THIS CAUSED A CONTUSION OF THE RIGHT LEG AND FOOT.
EE STEPPED WRONG ON 3-1-01 HE WAS NOT CONCERNED AND DID NOT WANT MEDICAL TREATMENT ON 6-5-01 HE ADVISED THAT HE WANTED TO GO TO THE CLINIC JUST TO HAVE HIS KNEE CHECKED. HAD MRI 6-18-01.
EE WAS BEATING SLAG OFF WITH A HAMMER. SLAG FLEW UP UNDER HIS SAFETY GLASS AND INTO HIS LEFT EYE.
EE WAS LEANED OVER THE RAIL OF THE DREDGE PRYING ON A BAR. HE STRAINED HIS LOWER BACK. HE WAS ALSO PULLING CABLE.
EE DEVELOPED TENDONITIS IN HIS RIGHT HAND. DUE TO REPETITIVE MOTION. HE IS A LOADER OPERATOR.
