While repairing a gate post, three employees, using a service truck and a Cat 950B loader, "to reshape the post". While the loader was moving into position its brakes failed and overran his intended point and an employee.

NOTHING OUT OF THE ORDINARY. BRIGHT SUNNY DAY. NOTHING TO TRIP ON/OVER. EMPLOYEE ENTERED THE CAP HOUSE, BENT OVER TO PICK UP A BOX OF CAPS (APPROX. 15 LBS). HE FELT A SHARP PAIN AND WENT DOWN TO HIS KNEES AND COULDN'T GET UP.
