Miner was working near the motor and drive belts to unplug a shoot. During troubleshooting they had the operator fire it up to see if it was good to go, during startup the turnbuckle that supports the motor broke causing the motor to swing striking EE in the lower back.

As we were attempting to run coke through the system to empty the tank it was noticed that the coke was still smoldering and smoking, so the attempt to empty was abandoned, the fire suppression system was deployed, and the fire department called.

No actual flames were found, but a hot spot of smoldering coke was found on top of our coke tank. Water was used to put out the smoldering coke, it was later discovered that a steel beam underneath the smoldering coke and some material inside the tank had reached a temperature of 1200 degrees. The fire suppression system was deployed and the fire department called.

The C92 conveyor belt had a slow speed sensor alarming. To remedy the issue employee was attempting to replace the drive belts by rolling them over the sheave. Employee did not follow proper lockout tagout procedure. Due to the conveyor not being locked out, as they were rolling the belts on the sheave the conveyor started and caught their middle and ring fingers causing injury.

A belt heater malfunctioned causing the C90 conveyor belt to catch on fire. A miner noticed the fire and tried to extinguish it. Once two extinguishers were used the miner could not put the fire out, so EE proceeded to call for someone on the radio to get the fire department on site. The Buffalo fire department came and extinguished the fire.

Miner was walking out of a doorway carrying a part the EE was going to do repairs on, when EE slipped on some ice. Slipping on the ice caused EE to fall to the ground and drop the part EE was carrying on EE's right ring finger.

The brick rig was in use in the kiln to support brick being installed. Pogo sticks are set on the rig to the brick to keep pressure on them until the brick ring can be keyed. The brick rig shifted causing the pogo sticks to lose tension resulting in the brick above to fall, severing the last 3/4inch of EE's finger tip.

4/22/19-In morning noticed that petroleum coke spill between 2 kiln bldgs smoldering. Location of spill made access initially difficult. Was evaluated how to access, extinguished ASAP 4/22/19. MSHA was not notified or an initial 7000-1 report done as issue was not viewed as fire or any risk of combustion. Failure to report resulted in paperwork citation #9382974 issued on 7/29/19.

Kiln had rock in it, as the kiln turned, exposed hot rock. Process control system allowed gas back into kiln which ignited as soon as hot rock was exposed when kiln turned. EE not wearing PPE required**

Lime Plant maintenance employee got lime dust in eye. The weather was windy. EE was wearing safety glasses with side shields. EE flushed eye several times with no relief. Supervisor told EE to see a doctor. The doctor confirmed a chemical burn from the lime dust and gave employee prescription eye drops for treatment of any infection.

On the above listed date, EE was in the process of dumping K-rock and the trailer tipped on its side. He hit his head on an unknown object inside the truck cab. The trailer, a 2001 Vantage, was towed away. The tractor, a 1996 Freightliner, has been inspected. It passed and is in accordance with 49 CFR part 396.

Employee was transferring oil into a pump and moved the oil barrel away from the wall in order to have a better position on the barrel. The barrel slipped from Employee's hands, crushing Employee's left little finger between the oil barrel and the wall.

COMPRESSED AIR WAS BEING USED TO DISLODGE ICE FROM A PIPELINE. AS THE ICE BECAME DISLODGED THE COMPRESSED AIR AND ICE WERE FORCED OUT OF THE END OF THE PIPE. THE COMPRESSED AIR EXPANDED AND TH E FORCE OF THE EXPANDING AIR KNOCKED THE INJURED OVER. INJURIES INCLUDED DISLOCATED SHOULDER, BROKEN CLAVICLE AND MINOR CUT.
