Employee slipped on the concrete floor and fell on EE's left side. Physical therapy and restrictions were prescribed for the employee. Injury became reportable 4/26/2024 when the MRI validated the injury was work-related.

Employee pinched finger between boom of service truck and cap/bar on crusher. EE received 3 stitches and broke the tip of finger.

Employee was moving a piece of steel and the steel readjusted and pinched the employee's finger. The pinching caused an open wound in the finger which required stitches.

EE was removing a V-belt on the crusher using a utility knife. The knife cut throught the belt faster than anticipated and struck ee on the thumb of his right hand resulting in a small cut that required three stitches. EE was wearing gloves at the time of the incident.

A CO-WORKER HAD POSITIONED A WEAR PLATE ON A WELDING TABLE ACROSS FROM WHERE EMPLOYEE WAS WORKING. THE FIRST EMPLOYEE PROCEEDED TO STRIKE THE PIECE OF WEAR PLATE WITH THE POINTED END OF A WELD ER'S CHIPPING HAMMER TO CHECK FOR HARDNESS. THE POINTED END OF THE HAMMER BROKE OFF, FLEW ACROSS THE TABLE AND EMBEDDED ITSELF INTO THE VICTIM'S FOREARM.

EE WAS UNLOADING STEEL FROM A PICK-UP TRUCK WHEN HE FELT A PREVIOUS INJURY TO HIS THUMB OPEN UP AND BEGIN TO BLEED. EE HAD CUT HIMSELF AT HOME AND HAD THE LACERATION STITCHED. THE STITCHES HAD BEEN REMOVED AND THE WOUND RE-OPENED DURING THEEMPLOYEE'S WORK ACTIVITY.
