Employee was removing unnecessary tools and trash from the back of the service truck. While walking back to the service truck after placing trash in the dumpster, "located next to the shop approximately 16 feet from the back of the truck", he felt a pop in his right knee.

Employee was assisting in removing curtains. While pulling the curtains out of the way, he twisted and pulled his knee. He felt a slight pain and continued to work. Later that same day, while walking across the yard going to the break room, he felt something tear or stretch and had instant pain. Employee still felt it was nothing and continued to work through the week.

EE WAS TROUBLESHOOTING A PROBLEM WITH A MOTOR THAT PULLED A CONVEYOR. WHILE TROUBLESHOOTING, THE PANEL WAS REMOVED FROM THE CIRCUIT BREAKER CONTROLLING THE CONVEYOR. THIS WAS CHECKED WITH THE POWER OFF THEN WITH THE POWER ON. HE INSERTED PRPROBES CAUSING AN ARC, RESULTING IN A FLASH OF HEAT & FLAME RESULTING IN SEVERE BURNS TO EE.
