EE STATED HE WAS LIFTING A 1/4" SECTION OF METAL 4' X 8' WHEN HE FELT PAIN IN THE LOWER BACK. THE PAIN WORSENED AND HE WENT TO THE DOCTOR ON 8/24/05. EE RECEIVED PRESCRIPTION & WAS RESTRICTED FROM LIFTING OVER 20 LBS. THE RESTRICTION WAS LIFTED ON 8/29/05. (NO LOST WORK DAYS.)

EE WAS WORKING ON DREDGE DRAINING WATER FROM GEAR BOX. AS HE ROSE UP AND BEGAN TO LEAVE THE PUMPROOM HE STRUCK HIS HEAD ON THE STEEL CROSS FRAMERESULTING IN 8 SUTURES. HE HAD TEMPORARILY REMOV ED HIS HARD HAT DUE TO THE TIGHT CONFINING SPACEOF THE WORK AREA.
