Employee had just put a pin in the ripper and as he was standing up the ripper shifted causing the chain from the boom to drop hitting him on the shoulder.
He was entering dozer and hit his head on a converter mounted in it.
He was climbing down ladder & stepped on a loose dirt clod & twisted his ankle. X-rays showed he cracked a bone in his right ankle.
HELPING MECHANIC LOOSEN NUT AND ALL THREAD ON THE D11 DOZER. NUT LOOSENED ATT AT ONCE AND ALL THREAD CUT HIS FINGER.
IT WAS MUDDY. HE WENT TO STEP OVER A HOSE, MISSED AND TWISTED HIS KNEE TRYING TO KEEP FROM FALLING.

HE WAS MOVING THE LIGHTPLANT AND WENT TO UNHOOK IT AND THE JACK KICKED OUT HE CUT HIS FINGER ON HIS LEFT HAND.
EE STEPPED DOWN TO GET A SCREWDRIVER. THE GROUND WAS MUDDY AND HIS FOOT TWISTED AND LEG DIDN'T. HE BROKE HIS ANKLE.
