The ee was sitting in a 771 haul truck being loaded with a 988 loader with spoils from the pit floor to be hauled to the dump. EE said that the truck rocked and he heard a pop and felt a pain go in his back.

TWO MEN WERE CHANGING TOGGLE BLOCKS ON PRIMARY JAW. THEY WERE TAKING THE NUTS OFF OF THE TENSION ROD, THE ROD SNAPPED PROJECTING THE SPRING WASHER INUT AND THE BUSTENED OFF. ONE MAN EE, WAS ST RUCK ON FOREHEAD. HE WAS TAKEN TO HOSPITAL AND RECEIVED 8 STITCHES.
