A loader operator thought EE had a leak on a 980C loader. EE parked the loader and climbed into the front right fender well to inspect for the leak. As EE was exiting the area, EE dislodged EE's hard hat on the fender, and it fell to the ground. EE bent over and picked up EE's hard hat, raised back up; and struck EE's unprotected head on the fender.

While removing hydraulic pump from machine, the lifting chain/strap caught and subsequently released causing a sudden upward shift of the chain.

Two employees were tightening bolts on a hydraulic rock breaker. An employee was handling a wrench, the wrench slipped off of a bolt, the wrench struck the second employee in the mouth resulting in an upper lip laceration.

Employee was preparing to work on chute at end of #8 conv. While he was standing on the catwalk a part of the catwalk floor gave way causing him to fall onto the catwalk. While he was falling he grabbed the chute and it cut his left hand requiring 8 stitches.

EE WAS CLEARING AG-LIME OUT OF THE ROLL CRUSHER. THIS EQUIPMENT WAS LOCKED AND TAGGED OUT. THE MATERIAL WAS CLEANED OUT FROM UNDER THE ROLL CRUSHER AND NEEDED A SMALL AMOUNT REMOVED FROM THE T OP. WHEN THE MATERIAL CLEARED THE TOP, THE ROLLSPULLED EE FOOT DOWN WARD MASHING HIS BIG TOE AND 2ND TOE REQUIRED STITCHES.

THREE EMPLOYEES WERE SENT TO CHANGE A BROKEN SCREEN, INJURED WAS CUTTING A PIECE OF METAL OFF THAT IS USED TO DIVERT MATERIAL TO A CHUTE. AFTER THE METAL WAS CUT LOOSE, HE PROCEEDED TO MOVE TH E METAL OUT OF THE WAY WHEN THE INJURY OCCURRED.
