EE was entering the building on EE's way to EE's office when EE tripped on the steps going up to the door. This was on Friday. We considered it first aid. On Tuesday EE went to the Clinic for an X-ray. X-ray showed a fracture in EE's right foot. EE came back with restrictions. This is when it became an MTI and recordable to MSHA

EE was removing a wire from the #5 rotary valve when the valve turned and pinched EE's left middle finger, removing the fatty tip of the finger.

Employee was working in #1 kiln baghouse helping to change bags. The employee was kneeling to pick a broom up off the tube sheet. While kneeling the employee overextended self trying to reach the broom and employee's weight shifted to left arm and wrist twisted on the blow down tube that employee had ahold of. Employee felt a pop at this time in left wrist.

Employee was changing silo hammer line valves when EE experienced a sharp pain in EE's Left shoulder. Employee reported the incident but did not seek medical treatment at that time. Employee tried home self-care but the shoulder did not get better. Employee sought medical treatment and was referred to physical therapy, making it a reportable injury.

Employee was on knees on FM1 beating the hatch loose on the secondary end so we could drop and pull this hatch to spin balls out. As employee was swinging down, index finger on right hand was pinched between the sledge hammer handle and a liner bolt that was just to the north of the hatch.

Employee was pulling an unloading hose (4" diameter) from Bay #2 to Bay #6 the hose caught on an I beam. IE turned to see what the hose was struck on, pulled hard, turned to continue toward Bay #6 and tripped over another hose. IE fell on left knee. Landed on concrete that caused a contusion to left knee.

Employee was walking to EE's car after clocking out EE fell going to EE's car and dislocated EE's left pinky. There had been a heavy down pour minutes before the incident. Employee's pinky was put back in place and EE returned to full duty the next day. On 7/27/2020 EE was re-examined and the pinky was not set correctly. Pinky was reset and employee was placed on restricted duty.

While attempting to blow out debris from the direct burn line fuel splashed out onto my PPE and unknowingly on my boots. After a couple of hours my foot was tingling and would not stop. I pulled off my boot and noticed material had soaked through my boot to my sock. Pulled off sock found that it made contact with my foot, causing chemical burn.

Employee was using a box cutter knife and placed it on a pallet of sand knee high with the blade exposed. The employee walked away and as ee walked back toward the pallet ee ran knee into the knife causing a laceration of the left knee.

Employee was operating a 12 ton electric hoist. EE placed EE's hand on top of the block while the hoist was traveling up. EE's pinky finger on EE's right hand was pinched between the shiv and the cable lacerating the top of the finger and breaking the pinky finger.

Employee was using a standard metal sledge hammer to remove the teeth off of a loader bucket when a piece of the tooth fractured and struck the employee in the arm. The piece of metal hit the employees left forearm causing a laceration and the metal piece to stick in the arm. The metal piece that fractured was approximately 1/4" (long) X 1/8" (wide) X 1/16" (thick).

Employee was attempting to adjust a fan damper when the ladder they were on slipped and they fell to the ground. As the ladder came to a stop at the bottom the employee jumped to the right and felt a pain in both ankles.

Employee was working in the #1 kiln dust collector re-bagging. EE noticed arms becoming irritated so put on a long sleeve shirt to keep dust off. The following morning reported arms at the bend of the elbow were showing signs of irritation.

Employee was working on a pickup truck in the maintenance shop, helping another mechanic to lift the bed off of a 1/2 ton pickup truck. Employee stated that ee "felt a "pop" in lower back but felt like it would go away". The next morning the employee reported the incident to supervisor and that back felt stiff.

Employee tripped and fell while measuring a silo. When EE lifted the cover, the silo was blowing dust out. EE turned head quickly to get out of the dust and tripped. EE fell forward onto the silo lid that is a 3'X3' box 18" off of the ground. EE's shoulder hit the lid first then the right side of face.

Employee was walking down a flight of stairs wearing cleated over boots when EE lost their footing. The stairs were made of a perforated grating for grip. The cleat caught in the grating and EE's momentum caused EE to lose footing. EE hit the bottom but said was ok. EE was sent to the clinic as a precaution. EE was given a 25 lifting restriction until 2/2/2015.

Employee was attempting to seat a valve on a diverting plate by striking it with a hammer. EE missed intended target and EE's hand struck an adjacent piece of metal lacerating the pinky finger requiring 3 stitches.

Employee was operating a 988F end loader. The air suspension compressor for the operators seat had a blown fuse. The repetitive bouncing through normal operation of the loader caused lower back pain.

While doing routine work employee felt discomfort in left shoulder. He was sent to a health clinic for evaluation and was returned to work the same day with no restrictions. After continued discomfort, surgery was deemed necessary on 1/29/2016. This did not become reportable until surgery on 01/29/2016.

While pulling rods out of the Raw Mill an employee got their fingers pinched. The crew was using a cable sling to bundle mill rods together and hoist them out of the mill. As the sling came up the rods rolled and one of them slid on top of the employee's hand pinching the two middle fingers on the left hand.

A laborer was attempting to check an oxygen bottle to see if it had anything in it when he was burned. He was standing on the right side of the bottle and used his right hand to open the bottle. There was a flash and the employee's right side and right forearm were exposed to a flash fire. The burns covered an area from his wrist to his upper bicep.

Employee was walking between cooling tower & tank 7. The plant had endured numerous snowfalls and multiple days of sub-zero temperatures. Snow had been cleared but employee stepped on a patch of ice, slipped and landed on his hip, also hitting his head in the fall. Employee taken to physician, placed on restricted duty until re-evaluated. Released to full duty 2-1-2014

Employee was attempted to open a diverter gate on #2 kiln clinker drag conveyor. The gate is operated by a 4' handle. When he applied tension to the handle, it broke free unexpectedly and struck him in the forehead. He was taken for medical treatment, received 5 stitches, and was returned to work with no restrictions.

Employee was cleaning debris from a Raw Mill feed hopper when the seat of his pants got wet from contact with material on the wall. Employee later complained of an irritation of his right buttock. As he drew the shaft out, he nipped the tips of three fingers on his right hand cutting the tip of his R middle finger, treated on site, sent to Dr. for further evaluation where he received a few stitches on his fingertip and placed on left handed duty only until follow-up appt on 7-14

Employee had filled a bucket w/ CKD dust. When he set the bucket down, impact w/the ground caused the dust to slosh out of the bucket, getting in his eyes. On-site personnel flushed eyes. Employee sent to phys. for further irrigation.phys. removed dust particles from eyes, employee was given prescription eye drops & returned to work w/ no restrictions.

Employee was cleaning hard build up from the bottom of #1 Kiln Clinker elevator with small chipping hammer. Employee was wearing approved safety glasses, and dust mask. When he activated the handle on the chipper the 5/8" air hose came loose from the hose barb on the quick coupling, and struck the employee in the right eye.

Employee was descending ladder from a haul truck and stepped off ladder on to a baseball sized piece of shot rock on the ground with his right foot. This caused his right knee to pop and immediately he felt pain. The haul truck was located West of the fuel pump station.

During the unloading of a Montana Minivan that was loaded with parts stolen from the storage barns an employee's right hand was pinched between the forks of a pickup truck and a clinker cooler spindle when the spindle rolled. The employee said that his right hand was throbbing and he had minor abrasions on the top of his hand.

Employee was using Flexane 80 putty to seal around bolts on the SPF hopper screws. Vapors being released from product caused some type of allergic reaction on the arms of employee. Employee noticed itching and removed himself from area. A poison ivy type rash developed on arms.

Employee was exiting the hopper door on the #2FM, #1 DC screw when the stepladder he was using tipped over and he fell approx 5ft to the floor. He landed on his back and struck the back of his head on the floor. Employee was evaluated at Company Physician. Evaluation determined head contusion, no other injuries were noted. Employee was placed on a work restriction of no climbing.

There was fire inside the Pier 6 switchgear room on #1 kiln at 12:36 on 10/18/10 that took down the #1 kiln. The turbosonic compressor breaker caught on fire and tripped the main feed in the sub at Pier 4. The smoke detector in Pier 6 switchgear activated and gave the location of the fire in the control room.

Employee was using a chain hoist to lift the cover from a hoist gearbox on the P&H crane. When the cover broke free from the gearbox, the beam above jumped slightly, causing dust to fall into the employee's right eye as he looked up. Employee was treated by company physician and returned to work the same day.

Employee was replacing gudgeon shaft and hangers in a 9" conveying screw. He was using a bar to lower the screw back down into the housing. The bar slipped and caught his right index finger between the bar and the screw housing. Fracture to tip of right index finger.

Employee was at the top of the ramp, shaking out gallon plastic bags that are used for lab samples, and he accidentally hit his right index finger on the top of the handrail. A few days later his finger because swelling and became very painful. His range of motion is also impaired. Employee reported this accident on 5/27/2010 to his supervisor and on 5/28/2010 to the HR Manager.

Employee was packing cement and one of the packing spouts activated on its own causing a stream of cement to hit the employee and dust entered the employee's right eye. Employee's eye is red and irritated. Employee was treated and issued eye ointment and patched.

Employee was using an air powered impact wrench and a universal joint and a 1 5/16" socket. While reaching to remove the bolt his finger energized the trigger of the wrench and the socket flew off the universal joint striking employee in the center of his upper lip.

Employee was standing beside the electric water pump while it was running when the 2" ball valve came apart on top of the pump. He was sprayed with water and then the cam lock end of the discharge hose came apart and hit him in the nose. The cam lock struck him in the safety glasses and nose. He received a cut on each side of his nose and his nose was broken.

Employee was hooking up the fuel cart between the shop and the silos and as he was cranking up the dolly, the tailgate on the truck fell and struck employee's back and right elbow. Employee reported pain and saw comp physician on 12/21/09 for evaluation. Employee was later scheduled for physical therapy on 1/19/2010 and was not previously on restrictions.

Employee complained at shift change that his left eye felt like there was something in it. At 01:30 he reported to the shift foreman that his eye was really starting to irritate him. Used eye wash, but did not work. Injury visited the doctor to look at his eye. Dr. found a piece of dirt under the eyelid. The doctor put some drops in his eye.

Employee was entering oil shop through the north door of the maintenance shop. Door was partially raised because the Oiler was pumping used oil from mobile equipment oil drain pan to used oil storage container in the oil shop. Employee thought he had ducked low enough to clear the door way, but he didn't and hit his head into bottom of door, which caused his neck to jam.

EE was coming down the P&H crane stairway and when he was almost to the bottom of the stairs, he heard a bang from the cement elevator on #2FM, he looked over that way and misjudged the last stair and landed on the ball of his left foot, a little past the last step. This is larger than the rest of the steps. When he did, his left ankle started to hurt.

He stood to the side of the tank and regulator reached his right arm and hand across the front of the regulator and slowly cracked opened the oxygen tank valve. A loud sound like a shotgun blast occurred when the back of the regulator blew apart. Pieces of the regulator struck his lower right forearm causing it to bleed.

While welding, a hot piece of slag burnt through EE's glove and burnt his hand. He took his glove off and the skin where he was burnt came off. The burn is on his right hand on his pinky knuckle and his ring finger knuckle and is about 1" long and 1/2" wide. He went to the first aid room and cleaned the burn, put on antiseptic, and dressed the burn.

Employee was pulling old motor leads from #1 ID fan drive - cutting the wires off with a Sawzall. I was holding the large wires (3/4" copper stranded) in my left hand and running the saw with my right hand. I was bending the wire with my left hand when the saw drew my hand in. I was wearing my cotton work gloves.

Clinker Elevator for #2 Kiln would not run. Found out the Chute going to cross apron was plugged up with dust and large chunks of clinker. Two co-workers and I were trying to get the chute unplugged using a Jack Hammer. While I was using the Jack Hammer inside of chute the material broke loose and pinched my finger between elevator housing and the jack hammer.

There was some smoke leaving the #2 coal tank that couldn't be seen from the ground. We notified MSHA at their 800 number to report a possible fire in our #2 coal tank. We then ran a fire hose up to the top of the coal hopper from the fire hydrant. The foreman and a laborer flooded the coal inside the tank with fogging spray.

EE was walking on the catwalk used to sample trailers. After checking the load his foot slipped causing him to fall and hit his knee. His knee had a small scratch and 20 minutes later he had fluid on his knee causing it to swell up to the size of a golf ball. Failure to use the stairway provided. Ladders are there only for emergency exit off the catwalks.

He was cutting balls out of the discharge screens in #1 Raw Mill with the plasma cutter. A hot piece of slag rolled down into his glove. causing a burn to rt palm below the thumb. On 4/22 was sent to the dr., put on restrictions and given an antibiotic for his infection, which resulted in an allergic reaction, and he was sent back to the dr. on 4/29.

Employee was alternating between using the wrenches and using a laser aligning tool, and was not wearing gloves at the time. One of the wrenches slipped off and open end of the wrench punctured the palm of his left hand, requiring six sutures to close the wound.

Employee was using a pocket knife to push in a jog button for the belt #1 at the crusher to help in determining the problem with the scale readout. As he was pushing on the knife the blade came closed on top of the employee's right hand index finger at the knuckle area. This resulted in a cut approx. 1/2" long and deep enough that it was necessary to go to doctor for stitches.

Contractor was welding on the #1 Kiln Shell. Employee was performing his normal duties, causing him to walk on the catwalk beside the kiln. A small piece of hot slag, approximately the size of a pellet, rolled around the kiln shell, was deflected off the pier and landed on the back of his neck. This caused 2nd degree burns to the back of his neck.

Another EE rolled the bucket down to make it easier to pull out, not realizing the EE's foot was under the front edge of the bucket. EE suffered a brusie/contusion to the top of his right foot when it was pinched between the bucket and the floor.

They then removed their locks, stood back away from the cooler arm assembly & tried to bump the jog button to move the crank arm. The steel bushing broke and struck employee.

HE REACHED BACK TO GRAB THE SAFETY HANDRAIL BUT MISSED AND FELL APPROXIMAT ELY 15 FEET TO GROUND LANDING ON HIS TAILBONE. 911 WAS CALLED AND HE WAS TAKEN TO HOSPITAL. ON RESTRICTED DUTY.

WHILE ATTEMPTING TO REPLACE THE TOP COVER TO THE CEMENT COOLER. THE COVER WAS CAUGHT BY THE MOVING SUPPORT ASSEMBLY FOR THE RIBBON SCREW. THE COVER TWISTED IN THE EMPLOYEE'S HAND CAUSING HIS H AND TO BE PINCHED AND CRUSHED BETWEEN THE COVER AND THE OUTER RIM OF THE ACCESS HOLE.

EE ENTERED THE 32 RAW MILL TO BURNOUT THE HATCH. THE MILL WAS HOT AND HUMID. THE EE WAS USING A TORCH WHICH CREATED ADDITIONAL HEAT. HE BEGAN FEELING OVERHEATED AND HAD DIFFICULTY BREATHING. H E EXITED THE MILL. HE PASSED OUT FOR 5-10 SECOND. EMT WAS CALLED AND HE WAS REMOVED TO THE LOCAL HOSPITAL.
