Employee was removing a steel wear plate in a loader bucket when it came loose and struck employee in the right eye/cheek

Employee was assisting in replacing a conveyor belt when the reel device struck employee in the arm.

Employee was removing a screen from a screen deck and contused his hand. Incident was originally not considered reportable. Employee was referred to a specialist after pain. Found to be an aggravation of arthritis and was placed on confirmed lifting restrictions on 8/15/13. Date incident became reportable.

Employee was climbing the ladder to access his haul truck when he bumped his elbow on the access platform.Incident did not meet reportability requirements until 8/19/09 when employee was issued work restrictions following a cortisone injection.

Employee was diagnosed with work related illness.

Employee was pulling torch hose to work area when he slipped and fell to same level.

Employee was walking down the stairs at the scale house when she slipped and fell. She initially had a bruise which turned into an infection that required treatment on the 6th of March.

Pedestrian had an automobile accident on a street adjacent to our property. He was reported missing on the accident date and was discovered down an embankment near an access road on the outskirts of our quarry. He appearantly died of overexposure. The victim was a tresspasser and this event is non-chargeable as a mine related incident.

Employee was pulling on a water hose when he wrenched his lower back.

Employee hit a bump in an LHD and jarred his back.

Walking to pick up and slipped on stone dust which twisted his left knee.

Employe was using an oxygen and acetylene torch when he dropped the torch and it burned his hand!

Employee was assisting in a maintenance project when a metal shard struck him in the chest. (REPAIRING CRUSHER - COWORKER WAS USING HAMMER.)

Employee stepped up onto ladder and slipped, catching himself with his shoulder and arm.

EE SLIPPED ON THE ICE AND FELL ON HIS LEG.

DRIVER WAS DRIVING ON HAUL ROAD WHEN RIGHT TIRE CONTACTED BERM, PULLING HIM INTO AND OVER BERM. TRUCK ROLLED DOWN EMBANKMENT LANDING IN WATER. DRIVER RECEIVED 8 STITCHES TO LEFT ARM & SMALL C UT OVER EYE.

EE WAS DRIVING MANLIFT TO JOB SITE WHEN HE HIT A HOLE THAT CAUSED HIM TO WHIP TO ONE SIDE.

EE WAS OPERATING A SMALL UTILITY LOADER WHEN HIT A DIP INT HE ROAD WAY JARRING HIS LOWER BACK. HAPPEN ON THE 16TH BUT DID NOT GO TO THE CLINIC UNTIL THE 17TH PER EE.

EMPLOYEE WAS OPERATING HAUL TRUCK ACROSS THE YARD WHEN HE HIT A HOLE THAT CAUSED THE MACHINE TO BOUNCE AND JARRED THE EMPLOYEE'S BACK.

EMPLOYEE JUMPED FROM 65 TON TRUCK WHEN THE ENGINE CAUGHT FIRE. FIRE CAUSED BY POSSIBLE FAILURE OF HYDRAULIC BRAKE LINE.

EMPLOYEE WAS WALKING ALONG SIDE OF WATERTRUCK WHEN OFFSTEPPED A ROCK, RESULTING IN A TWISTED KNEE. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION UNTIL 7-18 WHEN THE DR. PUT HIM ON RESTRICTED DUTY.

EMPLOYEE WAS GATHERING SMAPLES WHEN THE BUCKET FELL OUT OF HIS HAND. IN HIS ATTEMPT TO CATCH THE BUCKET HE SNAGGED HIS HAND ON A JAGGED PIECE OF METAL CAUSING LACERATIONS TO HIS INDEX FINGER OF HIS RIGHT HAND. SUTURES WERE REQUIRED.

EMPLOYEE WAS ATTEMPTING TO CLOSE A GATE TO RE-ROUTE MATERIAL WHEN HE FELT A STRAIN IN HIS LOWER BACK.

EE WAS MOVING RUBBER CHUTE LINERS WHEN HE HERNIATED 2 VERTABRAE IN HIS BACK. EE DID NOT REPORT UNTIL 3-8-01.

EMPLOYEE WAS USING A PRY BAR TO LOOSEN A CHUTE TO REMOVE IT. THE CHUTE MOVED SUDDENLY AND THE PRYBAR STRUCK THE EMPLOYEE ON TOP OF THE SHOULDER.

WALKING DOWN STAIRS WHEN HE TWISTED HIS BACK.

EE WAS CHECKING CONTACTS ON A MOTOR CONTROL STARTER WHICH HE HAD JUST DE ENERGIZED BY MEANS OF A CIRCUIT BREAKER. WHEN CONTACTS WHERE MANUALLY ENGAGED ARCING OCCURED AND EE WAS HIT BY THE FLAS H CATCHING HIS CLOTHES ON FIRE.

LIFTING HEAVY EQUIPMENT OUT OF A PICK UP TRUCK AND SET IT ON THE GROUND IN AN AWKWARD POSITION.

EMPLOYEE WAS CHECKING HIS BATTERY, AND AS HE WASREACHING TO CLOSE THE COMPARTMENT DOOR, THE BATTERY EXPLODED.

CUTTING BELT MATERIAL WITH UTILITY KNIFE WHEN BLADE BROKE CAUSING IT TO SLIP BACK AND CUT THROUGH HIS GLOVE INTO HIS THUMB WITH THE REMAINING PORTION OF THE BLADE RESULTING IN STITCHES.
