While bagging 50lb bags of rock dust the employee heard a pop in elbow. This was only reported to us on 06/06/2016 at 2:30pm by the employee.

Employee was bagging rock dust and became weak feeling and dizzy. Employee was brought into break room. He was complaining of numbness in right arm and weakness in his legs. Ambulance was called and arrived to pick up the employee around 12:00pm

Employee called in from home and stated that this condition had been bothering him for years. Employee thought he had a pulled a groin. Sent him from home to doctor. He chose to go to the emergency room. Attending Physician examined and diagnosed a hernia. Physician recommended that he see his personal doctor.
