Underground blaster was loading a second row of holes on the face and as EE reached up to the hole EE's foot slipped causing EE to fall into the face and cut right forearm on a rock protruding from the wall.

2 employees were changing troughing rollers on conveyor C1A located in the decline to the underground. The injured employee fell to the ground approximately 16 feet. The injured employee was wearing a fall protection harness and fall protection lanyard. Evidence suggest that the fall protection device was either not tied off or was improperly tied off.

2 employees were removing impact bed from a conveyor. As they lowered it to the ground, one employee released their end before the injured employee released the other end forcing employee's left hand to slam into their thigh causing the impact bed to rip a glove off, resulting in a laceration to the ring finger.

Blaster was using 2 crescent wrenches to loosen and remove the beveled fitting from the hydraulic fill port so the lube truck hose would connect properly. When pulling the wrench, it slipped striking the employee in the mouth resulting in a lower lip laceration and a broken tooth (top right lateral incisor). The employee went to the dentist on 5/21/15 to repair the broken tooth.

After tightening motor mount bolts at the primary, employee was uncoupling an air line hose from the back of the mechanic's service truck, standing directly in front of the coupler (that was 55" off the ground), when it released, the hose kicked back and up striking employee on the upper lip. Resulted in laceration with 3 stitches & released to return to work without restriction.

Employee reported a twinge in left shoulder while hand scaling during roof bolting cycle. The orthopedic had classified this as biceps tendonitis as well as advanced end stage degenerative changes of AC joint. Employee had continued to perform his job but states the pain has not subsided; so on 8/29/12 doctor has placed employee on restrictions and scheduled surgery.

Employee sampling for Quality Control to ensure compliance. The Employee lifted 5 gallon buckets of material to spread evenly into a sample pan & felt a snap in his shoulder. 5 gallon buckets of material weigh about 65 pounds. He didn't need treatment until now, his condition worsened. May 18 2011 the employee had surgery & missed his next scheduled shift on 5/19/2011.

The foreman, who had prior soreness in his back, was operating a dozer. At the end of the shift the miner could barely move due to increased discomfort. Two days later the miner sought medical attention where a doctor treated the miner for muscle spasms. The mine operator was not made aware of this reportable injury, through medical documentation, until 8/11/2009.

The scaling machine operator was descending the two step ladder of the machine, when the right foot made contact with a stone on the mine floor irritating the Achilles tendon. At 2:29 pm on 12/1/2008, the miner sought medical attention at a clinic where a doctor diagnosed the condition as tendonitis. The miner resumed his regular duties at 4 pm on 12/1/2008.

A WMC Inc. employee was removeing rollers from a conveyor below the C2A conveyor platform while a Vulcan Employee was removing an inspection cover from a motor on the platform above. The Vulcan employee lost control of the cover striking the WMC employee on his hardhat from a drop of approximately 10-15' striking several points of the conveyor structure on the way down.

Employee was using a torch to cut two holes in plate metal. The plate was on saw horses. When he straightened up, he felt a pain in his back. He was seen on 9/11 by a physician and was put on restricted duty at that time.

Employee was repairing feeder-breaker. In the process it required a flyght bar(500 lbs.) to be lifted in order to get to another one. A power pack and hydraulic jack was being used overhead in the bottom pan of the feeder-breaker. The jack slipped and flew out striking employee on the safety glasses. The top of the lens cut into the eyebrow requiring sutures.

Employee was putting mats into the back. A mat (strap) was placed on the canopy of the bolter to be taken back to the back for installation.The mat was stabalized with a roof bolt as the compartment was rising. The mat slid on the canopy sliding down the bolt, pinching EE's finger between mat & bolt plate lacerating finger requiring sutures.
