Employee was under a F-250 pick-up finishing up tightening the tab bolts for the cross-member for the transmission. While in the process, one of the tab bolts spun loose and caught the EEs palm of EE's hand.

EE was using a torch to cut an opening in steel above EE. Either the flame from the torch or a piece of slag created an opening in the hydraulic line, which began spraying oil and caught fire. When EE turned away the flame/fire entered EE's welding shield. EE then jumped down from where they were working and caused pain in the right foot after landing.

Employee was placing guarding on the motor of 5 1/2 ft. crusher when a water line located in another part of the plant burst startling the employee and caused to seek shelter. After the employee found shelter and realized they were not in harms way, employee alleges pain in left shoulder. Sought medical care on 8/24/2020.

On 10/10/18 at approximately 1130 am the operator was operating the bolter in 16N22. Installing 8 foot roof bolts from the basket of the roof bolter. While inserting the 7th bolt, the lift cylinder bottom mounting pin boss had a structural failure causing the boom and basket assembly to fall approximately 18 feet, to the ground. The employee sustained multiple injuries.

Employee was loading holes at the face (1st level -9N21C) of the underground when a sharp rock struck the employee's right forearm resulting in a puncture wound that required 4 stitches. Employee was released to return to work with restrictions.

An employee exiting her office through a door stepped on a patch of ice which caused the employee to slip and fall to the floor. The employee was diagnosed with a possible left shoulder strain.

Employee was working on the deflector plate inside the U5 Discharge Chute, when the employee experienced pain in the right shoulder. On September 27th, the employee requested to see a physician. MRI revealed Tendonitis of supraspinatus & infraspinatus tendon as well as mild degenerative changes to AC joint. Employee was given PT and no lifting restriction.

A miner slipped on snow, spraining the right knee. He finished the shift and worked the next shift, the 2008 workforce layoff date. After recall on 3/9/09, the miner performed regular duties, but discomfort / swelling continued. On 10/13/09, a doctor recommended surgery. The miner chose to work until 12/3/09. The surgery date was 12/4/09, the 2009 workforce layoff date.

Employee was in the process of repairing a feeder breaker with a broken flight chain. In the process a flight was jammed. The employee used a sledge hammer to beat on it trying to free it. When he struck the flight with the hammer, a sliver of metal came off the flight and penetrated two inches into his right leg. The sliver had to be surgically removed.

Employee was exiting his truck at the end of his shift. He slipped while he was exiting and twisted and fractured his ankle. The case was under investigation until August 24, 2004 when it was deemed to be a work related injury.
