He was installing roof bolts in face. He went to retrieve another bolt, when a piece of coal fell out. Striking him in back of leg and ankle. (He received stitches on ankle.)

He was riding on golf cart. They back off road into x-cut, when a piece of material fell out and struck him on back, neck. **(He began missing work on 9-8-15)**

EE was helping replace a worn out screen, when installing the new screen, the pan raised up and smashed his finger between pan and I-Beam.

While driving inby on a 4-man diesel and looking in towards the belt, hit hard hat on the roof. Causing lacerations to head, received stitches.

He was driving down roadway, when a tractor ahead of him snagged the lifeline. It stretched, broke and the direction cone on lifeline struck him on cheek. (He received stitches)

He was moving a section of chute work on a cart. He had his finger on the outside of the chute. He pinched it between chute and impact box. (He received stitches)

He was on his knees, building a crib. when a piece of rock fell out, hit his shoulder and slid off striking his hand which was on ground. ( received stitches to hand)

HE was using a thread tapping tool. He put pressure on the wrench and the tool broke, causing him to fall forward , cutting his index finger on the tool.( he received stitches.)

He was standing next to a stack of overcast panels. They started to pull a panel off top of stack. The panel slid off top of stack, swung around and stuck him across legs. (fracture)

HE was replacing a joint fitting to waterline, when he turned to get parts off mantrip. He slipped and fell back , and landed on right elbow, which jammed shoulder. * started missing on 05/12/2015 with tendon tear*

He was working on shuttle car. He had lifted the cover to work in panel. He wedged cover in hog wire to hold it up. The cover fell and struck him on index and middle finger (Fracture of Index finger and cut on Middle finger)

Drilling hole to install roof bolt, Material fell out, slid off canopy, striking him on left ankle. (chipped a bone )

He was installing a 84" roof bolt, he went to bend to bolt ,slipped and bolt fell out of hole and struck him on forehead . (received stitches)

He was standing next to the miner, when a piece of material fell out and struck the top of his hard hat, slid down and cut his upper lip. (he received stitches)

There was an unplanned roof fall that occurred on #2 units 3rd roadway, crosscut #6. 18' long, 18' width, 6' to 8' height.

He was working to free up chain on chain hoist ( come-a-long ) One end was hooked on I-Beam, while pulling on chain, it came loose and stuck him on the upper lip. ( received stitches )

There was an unplanned roof fall, located on the 7th48 beltline, between crosscut 4 and 5. Length 12', width 6', thickness 5-6 ft.

He was driving shuttle car, he had arm laying on outside of cab, rib rolled off and struck his right arm.

While removing guarding from head drive, the guard sprang back striking him on the face, causing a laceration.( he received stitches.)

He was moving a piece of drive chain, chain slipped through his hand and cut the thumb on his right hand.

While putting bearings on a tail roller he bent over to pick up the bearing and felt pain in stomach. ***Began missing on 5/9/2014***

He was tightening up nut on scoop. A helper was holding backup with another wrench on top. The helpers wrench slipped and struck him in mouth. (He received stitches )

He was pulling down a piece of loose rock, when a piece fell out between roof bolts behind him , striking him on back. (Bruised back)

He was making spice on pinner cable, when his knife blade went through cut, cutting bottom of left thumb. (He received stitches)

While roof bolting, his pin plate got stuck on the ATRS, he reached up to clear it and the plate broke free pinching his right middle finger. ***stitches***

Employee was bolting #5 Left, just finished putting up his inside roof bolt. He dropped his canopy down and a rock fell from the supported top and struck him, pushing his head down into the canopy. ***Began missing on 1/16/14***

While pulling on a jakepipe and removing it from the tailpiece. He felt pain in left hamstring. ***He started restricted duty on 1/3/2014.***

Unintentional occurred on the 7th 48 beltline x-cut 12 , #5 entry 12' length x 12'wide x 6 thickness . ( This area was pinned with 72" tension rebar.)

Length 35', width 18' thickness 7' located on 8th 48 beltline, x-cut 30.5 , #5 entry.

While rolling up the curtain, a rock fell out that was hanging on top of the curtain, fell striking him on the right shoulder. ***He began restricted duty on 11/26/2013***

While throwing over the miner cable, he slipped and fell, he tried catching himself and pulled something in right shoulder. ***Began restricted duty on 11/19/2013***

There was a unintentional roof fall that occurred on #2 units 3rd belt at crosscut #8, (width 25'X 25' 10' height, in intersection) We are not planning on cleaning this fall up. This area was supported with 72" tension rebar roof bolts.

While installing roof bolts and hog wire to the mine roof, ee used his left hand to hold up the hog wire, the hog wire got wrapped around rotating steel, pulling his left arm around the steel.(causing the amputation of his left arm just below the elbow)

While taking about a belt splice, he was hitting a hammer against metal bolt cutters and a piece of medal came off hammer and lodged in the bridge of his nose. Dr. had to remove sliver of metal from nose.

While installing a roof bolt, gob fell out from between the pins, it hit the canopy first and rolled under striking him in the left calf. ***stitches***

There was a unintentional roof fall located on the 8th 48 beltline, between x-cuts 29 and 30, in the straight. It was approximately 50 ft long, 18 ft. wide and 8 to 9 ft high. 84 inch tension rebar bolts was used in this area.

He was scooping out header hole, (scoop did not have a canopy) Material fell out from roof, and struck him on left knee.( caused cut on knee required 3 stitches )

Pulled his foot out from between wheel and rail on supply trailer, slipped and fell on left hand, hurting left shoulder.( damage is a strained shoulder)

An unintentional roof fall has occurred in front of the #1 seal at the Route 1 Seals xc 27 entry #9 (6th 48 Main Return). We are not planning on cleaning it up, we are going to add 3 seals around fall. This area was supported with 72" tension rebar fully grouted roof bolts and additional cribs, props and timbers were added to the affected area.

While working on the unit tailpiece, EE dropped a metal skirt and tried to catch it. In doing so, felt something pull in his lower back. ***He began missing work on 10/17/2013.***

While hitting chisel with hammer, the hammer slipped off smashing hand between the two. ***Stitches***

An unintentional roof fall has occurred at xc 3 on the 8th 48 travelway, entry #6. We are not cleaning this fall up, we stepped the travelway over. The area that fell was approximately 20' x 20' and was bolted with 72" tension rebar roof bolts. The affected area was re-supported with 84" bolts, brow bars, cribs, props and timbers.

Was standing on opposite side of bolter, putting bits on stringer, when operator trammed bolter backward, running over top of left foot.

while getting in shuttle car, there was screen wire hanging down. The screen wire caught his right ear and cut it (received 5 stitches)

While dusting the beltline he bent down to remove a damaged bottom roller. When dragging the roller he felt a strain in his groin area, causing him to have surgery to repair a hernia. ***Began missing work on 8/26/13***

While pulling belt structure out of the beltline, he felt something pop in lower back causing pain.

While cutting a vertical belt pocket with a utility knife, the blade broke causing his hand to slip, cutting the top of his hand. ***5 Stitches***

While driving mantrip out, she was coming through a set of trapsets and started to slide. She struck the trapset and her face struck the steering causing a laceration. ***She began missing work on 05/02/2013***

While standing next to the roof bolter a piece of gob fell out from between the roof bolts striking miner on right leg. ***Stitches***

While cutting wood with a circular saw, he hit a knot and the saw jump back cutting his right thumb. *** He received 7 stitches ***

While in the squatting position, a rib rolled off behind him not striking him, causing him to jump up and twist his knee. ***He began missing work on 4-24-13***

Employee was on his right knee and using his left foot to operate the mantrip. His right foot got caught between the trailer and the mantrip causing multiple fractures. ***He began missing work on 4/9/2013.***

While connecting two drill steels, one kicked out and struck him in the mouth. He received a laceration on his bottom lip that required stitches.

Employee was working to drilling tongs when tripping out of the hole when his left hand slipped smashing his fingers. Left hand ring and middle finger were smashed causing employee to receive 9 stitches, meta carpal gloves were being worn at the time this occurred.

Loose gob fell out from between the rib pin and the rib, striking him on the right forearm, causing a laceration. ***Stitches***

While putting up a corner pin, somehow hit too much up pressure bending the drill steel and when rotated hit him in the left shoulder, causing stitches to be needed.

While loading waterline couplers he had his right hand on top of crate when a metal fire valve rolled off top shelf, smashing middle finger. ***Stitches***

While installing chain hangers at the face of #3 Unit's beltline, the line curtain became wrapped in the drill steel and pulled his right hand into curtain. ***He began missing on 12-4-2012.***

While moving the miners slack pile, he felt pain in stomach area. He began missing on 11/26/2012 due to a hernia.

Employee was bent over, waiting on car, when a piece of rock fell out between the roof bolts striking on back. (He sustained fractured vertebrae - no surgery is going to be necessary)

While picking up rock dust bags to load the wet duster, he felt sharp pain in lower back. ***He continued to work full-time without restrictions until 12-10-12.***

While loading the rock dust trailer, he was trying to lift himself up on the trailer with his left arm to pull the plastic off the bag. He felt his right shoulder pop and then give out causing all his weight to come down on it. He immediately was scheduled for a MRI and the results were positive for a tear in his rotator cuff. ***He began missing on 8-14-12 for surgery.***

While getting out of his tractor, he stepped into a hole and twisted left knee. He continued to work full time without restrictions until he was scheduled for surgery on 11/07/2012.

He was replacing a piece of metal on front of shuttle car, he picked it up and turned to put it in place, when he felt his left knee pop. **(continued to work without restriction , until 8-31-12 when he went off for surgery)**

While bolting #5 entry of #1 Unit, a rib rolled in and pushed his head into the tray of the bolter. ***He received 9 stitches on his forehead.***

While hitting a broken bit with a chisel and hammer, a slither of metal broke off and entered his left forearm. ***He needed 2 stitches***

While backing the bolter out of #9 Entry, the bolter began to sink in the bottoms and slide towards the rib. After hitting a big rock, the bolter jumped, throwing the operators foot out of the cab and smashing it between the rib and the bolter. He received a fracture in his left foot. ***He began missing on 06/04/2012.***

Employee was using come-a-long to pull a 3-wheeler out of mud hole, chain came loose, hitting him in forehead. (He received stitches to his forehead).

While running the miner on early coal, a piece of gob fell out from between the bolts striking him on the right shoulder. He continued to work full time without restrictions until the doctors ordered a MRI, where they found a torn labral. ***He started missing work on 07/09/2012.***

Miner operator was backing the miner out of the cut when a piece of rock fell out from between the pin and the rib, striking directly in the lower leg and ankle. ***Started missing work on 4-10-12 due to a fractured bone in leg.***

There was an unintentional roof fall located on #2 unit (x-cut 23) #7 entry , width 17' , length 20' height 6'. this area already had supplemental support added and was dangered off.

While kneeling down working on the miner, a piece of the coal rib rolled out striking him on the back and right shoulder. He continued to work full time without restrictions until the doctor put him off so they can do therapy and injections. ***He started missing work on 07/09/2012***

While bending an 84" tension rebar roof bolt, he felt a sharp pain in left shoulder. He continued to work full time without restrictions with pain until he was diagnosed with rotator cuff tear. ***He began missing work on 7-25-12.***

An unintentional roof fall occurred on the 4th 48 Belt Line between xc 38 - xc 39.

The crew was pulling a start box out of the crosscut when a braided steel cable got caught and pinned his leg between the cable and the corner of the rib. This resulted in a fractured right leg and ankle. ***Began missing work on 2-15-2012***

Using a sledge hammer to break up a piece of limestone, the rock broke and the hammer slipped causing left middle finger to strike rock. ** 6 stitches **

The roof bolter operator was cleaning the gob off top of roof bolter, his hand slipped and cut right wrist on piece of sharp slate rock.( had to have 4 stitches in right wrist)

While tramming the scoop he hit a hole, making him hit his head on the canopy, jamming his neck and right shoulder. He continued to work full time until he was put off work for physical therapy. ** Began missing work 01/30/2012**

While roof bolting, he was inserting sections of the drill steel into the hole that was drilled. He felt a sharp pain in his right shoulder. He continued to work full time without restrictions until 06-07-12, where it was determined that surgery would be needed to repair a small tear in his rotator cuff.

EE was dropping his cable from the hangers when he felt a sharp pain in left shoulder. He continued to work full-time until it was discovered that he had some rotator cuff problems. ***He began missing on 5-15-201*** He had surgery for rotator cuff repairs.

He was using an electric grinder on a belt drive, a small piece of metal got in his left eye. ( he was wearing safety glasses at the time) The piece of metal was removed.

An Unintentional roof fall occurred at 8:15pm on the #2 unit, xc #8, and in entry #7.

He was holding a jack pipe for the tailpiece and the pipe slipped off the foot board striking him over the right eye. *** Stitches ***

He was helping mechanic make a car splice and his belt knife slipped cutting his left forearm. *** stitches ***

He pulled golf cart through curtain and a shuttle car had just pulled through and was stopped, the golf cart hit the bumper and caused his head to hit steering wheel. ** stitches**

EE was making a bolt up with his back towards the rib. A piece of the rib rolled off stricking him in the back of the head pushing his head against the bolter. ** Stitches above eye **

Operator was getting ready to start pinning when a piece of the rib rolled off, knocking him down and pinning right leg. ***Began missing on 11-14-11.***

He was getting rock off of the conveyor of miner and the rock tilted, smashing finger between rock and the top. ** 4 stitches **

He was hanging the miner cable and felt pain in right shoulder. He worked full time until the MRI showed a small tear in shoulder. He started missing work on 2-13-2012.

He was helping the mechanic splice the bolter cable and slipped with belt knife, cutting his left thumb. **4 stitches**

While adjusting a line curtain a rock fell and struck him causing a laceration to the back of his head and a fracture of his L12 vertebrae.

He was trying to hitch two trailers together and had right hand resting on trailer when another trailer rolled back causing hitch to smash fingers against trailer.

He was pulling pins out of belt splice with a hammer and hit two hammers together. A sliver of metal from the hammer struck him in left arm. **Employee was given 7 stitches.**

The mechanic was in the middle of fixing the foot switch on a golf cart when he dropped his supplies on the cart engaging the tram pedal. EE was pinned between the golf cart and mantrip, deep bruising his right thigh. He was taken to the Doctor and was put off work until Sept. 2, 2011.

He was splicing miner cable with belt knife, hand slipped and cut finger on left hand. *** 2 stitches ***

Operator was using the boom of the bolter to dig out the mine floor to gain more height. He did not have the foot jack set so when he raised boom up the canopy dropped striking him on the head, jamming neck. Doctors thought it could possibly be a protruding disc and is put off work as of 8-4-11 during physical therapy.

He was stacking blocks to build a brattice when a piece of rock and coal rolled off and smashed right finger against block. *(6 Stitches)*

While installing lid of distribution box with a bar, the bar slipped out of the hole striking employee in mouth. *(stitches in lip)*

While building a brattice, he leaned over the trailer to pick a block up and felt a pain in his left shoulder. **He continued to work full-time w/o restrictions until he was put off on 01/17/2012 for surgery.**

While helping make a car splice he cut his left finger with belt knife. Resulting in 5 stitches.

While kneeling on left knee, another mechanic tripped over his extended leg, causing him to twist and strain right knee. ***He worked full time until 11-1-11 when he was scheduled for surgery.***

While pushing rock off top of miner, rock broke and fell into sharp edge of another rock cutting right forearm (required three stitches)

He was working on miner when he heard the top working. He jerked up quickly and felt a sharp pain in right hip and lower back. *** He began missing work on 5/11/2011.

EE was taking cover off of the top of roof bolter and the bolt broke off. He fell hitting his right shoulder on the rib. *** Started missing work on 5/5/11.

He was lifting a breaker assembly into a panel box when he strained his right shoulder.

He was trying to bend a 72" bolt to install it when he strained his left shoulder.

Was pulling a pin on the feeder and smashed his right middle finger.

Operator was picking up bits from miner in #8 intersection in the last open, when a rock struck him on left shoulder and back.

He climbed out of scoop, stepped in hole and twisted right knee. **continued to work without restriction until 4/26/12 when he began miss, due to surgery on his knee.**

Operator was getting pins from mid-section of bolter when a rock fell on him spraining his left shoulder. He continued to work until his last doctor's visit where they diagnosed an AC separation. Began missing work on 05/26/2011

Unintentional roof fall on #3 unit's roadway, xc 12 #6 entry.

While working on a panel for the surge belt he walked into a low hanging brow, jamming head and neck. ****He continued to work full duty without restrictions until he started missing work on 05-23-2012 due to surgery.****

The operator was trying to align chuck with pinner steel, he then started the rotation and the steel popped out and struck hand between index finger and thumb.

There was an unintentional fall of the roof on #2 unit's 3rd beltline between crosscut 9 and crosscut 10. The fall measured approximately 50' in length, 17.5' wide, and approximately 10' tall.

He stepped out of his low trac onto a piece of coal, twisting left foot.

The operator finished putting up his outside rib pin. When he moved from underneath his canopy, a piece of draw rock fell from in between roof bolts striking him on the side of his foot, resulting in a broken bone. ** started missing 9/30/10 **.
