IE was rolling the tarp off of McNay rental trailer #118 when the handle detached from the trailer causing the IE to fall to the ground, same level injuring left lower back. The IE did not seek medical attention until 11/15, when they were taken off work.

2nd Shift Scale House Clerk was giving paperwork to an over the road driver when EE suddenly collapsed and went unresponsive. CPR was performed until the ambulance arrived and EE was transported to the Hospital where EE was pronounced deceased.

IE was loading a 6" vacuum line into the back of a pick-up truck when they felt a slight "pull" or "pop" in lower back. The IE reported incident to supervisor on 6/22/2021.

IE was helping to free a bucket elevator leg. The leg, locked out at the time of the incident. Another employee using a pipe wrench to try and turn the shaft while the IE was trying to pull on the belt, when the pipe wrench slipped causing three of the IE's fingers to get pinched between the belt and a sheave. X-rays revealed a fracture to the tip of IE's middle finger.

The IE was moving a stack of pallets through the "speed door" in the Packaging Plant when saw dust came off of one of the pallets and into the IE's right eye. The IE was seen the following day by an OD who removed debris from the eye and prescribed antibiotic drops.

On 1/30/21 the IE was trying to free up material in the Mill 13 bin using a long rod to poke the material. EE felt a strain in EE's lower back and reported the incident to management. The IE continued to work until EE's doctor took EE off work on 5/18/2021

EE was filling super sacks with product in the packaging plant. As the super sack was going down the conveyor, EE noticed the sack was leaning. EE stopped and tried to push back on the sack and felt a pop in EE's left knee so EE was taken to ER. EE can't get into a Dr. to be released due to COVID-19 policies so EE has missed too many days of work.

IE was attempting to hook up a mobile air compressor by unhooking a stationary air compressor so that the stationary air compressor could be serviced. As they started to hook the 3" hose up, they tripped over the excess hose on the ground and fell. As they fell, left thumb hit the framework and caused a fracture to left thumb.

Removing belt from a motor and did not relieve tension on belt first. When right hand pulled the belt, left hand holding a screwdriver slipped and two fingers were pulled between sheave and belt. Two fingers were lacerated and very tips were fractured. Employee was retrained to loosen tension first to remove belt.

Employee was stepping out of rail car mover and when EE stepped onto the ground EE twisted EE's ankle. Went to be seen at clinic. X-rays were taken and EE was diagnosed with a slight sprain. Missed two shifts then back to work normally. Ground was level and no work conditions were noted as cause.

Employee collected a hand full of empty bags and turned full body to place the bags on the conveyor and at that time felt a sharp pain shoot from left rib cage area.

While trying to remove the dust collector bag, EE tried to open the top door on the dust collector with a clamp left on. At this point, the employee felt a pull in lower back. The employee was taken to a doctor, and was diagnosed with a muscle strain.

This employee and another employee were on the belt cleaning out a rock jam. The other employee was pushing the rock up the belt to this employee who was lifting the rocks out and throwing them over the side. They were just about done when this employee encountered lower back pain. Employee was taken by ambulance to the hospital.

A passenger vehicle was traveling south on Illinois 57 when the vehicle veered out of the lane and hit a retaining wall on mine property. The vehicle burst into flames, killing both the driver and a passenger. Neither of the individuals had any affiliation with the mine. Aside from proximity to the road, the mine had no involvement in the accident.

Employee was loading truck at Silo 33. Employee lowered load out chute into hole. Chute contacted lip of the hole rather than going down into it. Employee reached down with his left hand and pulled on chute to allow it in the hole. The steel rings of the chute pinched the employees left palm resulting in laceration approximately 2 inches long. First Aid was administered.

He explained that he fell while attaching the sack loops to the forklift. Access to forks on tow motor, and sack spout was being gained by a stack of 3 empty pallets, 4x4 boards, and a piece of plywood on top of the floor scale. As he stepped up onto the pallet configuration the plywood on top shifted causing him to fall.

Employee started descending from the packaging cart while carrying his large, personal portable radio in his right hand. The employee did not maintain the required 3-point contact as he descended and stumbled, chipping his upper left front tooth. Employee was taken to the dentist. The employee's mouth was x-rayed, there was no damage to the root or nerve and the tooth was filled.

The employee was using a pipe and a hammer to break up clumps of material in an outside hopper. The employee put one end of the pipe on a clump and went to strike the other end of the pipe. The hammer head glanced off the end of the pipe and the employee's hand that was holding the hammer hit the end of the pipe. The employee broke the second digit on his right index finger.

THE CUTTING EDGE OF THE BUCKET CLEARED THE RAIL BUT THE SKID PLATE UNDER THE BUCKET CAUGHT THE INSIDE RAIL CAUSING EE TO STRIKE THE LEFT SIDE CAB WINDOW WITH HIS HEAD AND SHOULDER BREAKING OUT THE WINDOW. INJURY RESULTED IN THREE SUTU

EE LOST CONTROL OF THE GRADING WITH HIS LEFT ARM AND THE FULL WEIGHT OF THE GRADI NG CAME OFF THE TRUCK AND, ACCORDING TO EE, HIS RIGHT ARM ABSORBED THE FULL WEIGHT OF THE GRADING. ATH EMPLOYEE ENTERED DOCK #6 AREA TO LOCATE TRUCK DRIVER TO REPORT PROBLEM WITH HIS TRUCK. WHILEINFORMING THE DRIVER OF THE PROBLEM, A UWI EMPLOYEE WAS IN THE PROCESS OF RE-ENTERING THE TRUCK TO COMPLETE UNLOADING THE FINAL TWO PALLETS OF PRODUCT WHEN THE ATH EMPLOYEE WAS STRUCK ON THE RIGHT FRONT BY THE TOWMOTOR. NEITHER EMPLOYEE SAW THE OTHER UP TO THE TIME OF THE INCIDENT.

THIS EMPLOYEE WAS UNDER MEDICAL TREATMENT FROM HIS PERSONAL DOCTOR FOR HEALTH CONDITIONS UNRELATED TO WORK. THE EE CAME TO WORK AND WAS IN THE PROCESS OF LOADING TRUCKLOAD OF 50 LB BAGS ON PAL LETS INTO A VAN TYPE OVER-THE-ROAD TRUCK. BEFORE LOADING COULD TAKE PLACE THE EE HAD TROUBLE BREATHING. EE WAS TRYING TO GET GROUND LIMESTONE MATERIAL OUT OF AIR SLIDE UNDER TANK 1 AT PACKAGING PLT. HE WAS PRYING ON A VALVE ARM ON AIR SLIDE WHEN PIPE SLIPPED AND HIT HIM IN THE LIP. UPPER LIP WAS LACERATED AND REQUIRED FIVE STITCHES.
