An employee on the ground level retrieved the tool and climbed 1 to 2 rungs of the ladder to hand the tool to the employee above. The employee appeared to lose consciousness and fell. The witnesses indicated that it appeared the victim was breathing. At 12:01pm, US Silica was informed that the victim had a heart attack and while at the hospital the employee died.

At approximately 10:30 PM smoke was observed in the B Pit Pump house due to the overheating of a mining pump motor. The local fire department was called and, at 11:15, the smoke was cleared, and a small fire was observed inside the pump and motor. The fire was fully extinguished at approximately 11:40 PM. This occurred in an unoccupied area of the mine.

While pulling a water hose across the floor it became tangled causing it to jerk. The jerking motion resulted in pain to the employees right shoulder. The employee went to the doctor on 10/08/2018 at which time employee was given restrictions and referred for physical therapy.

Employee was lifting up on an elevator safety gate with right arm. As EE lifted up the gate EE indicated that EE felt pain in right arm between forearm and bicep. X-ray and MRI done. Strain of arm ligament. Restricted duty-right arm and physical therapy. *Referred for surgery 2/9/17. Surgery took place on 2/17/17. Begin lost time on 2/17/17.

While using a bobcat to clean sand near a truck scale, an employee injured their rib (closed fracture), when the bobcat struck a protruding concrete footing. Employee moved the monitor station (using the affixed handle) from the lower to upper position, he injured his left elbow. The force observed to move the monitor was no more than normal to move the handle on the station. Poor positioning and ergonomics was identified as more significant risk factor during the investigation.

While grinding the sparks were hitting his stomach and somehow made it into the area under the face shield and safety glasses. Employee did not notice until 6pm and did not report this until 6:45am on 6/9/13

Employee manning a tagline while a piece of pipe was removed and lifted out by a sky track boom lift. Pipe moved slightly and the rope slipped through the left hand and index finger causing a laceration. 15 stitches were administered.

Employee was attempting to open a gate on the bottom of the rail car. The gate was stuck from residual materials from external sources, in addition the whether was below the freezing mark. The EE attempted to put more than normal force on the bar that opens the gate, and when the gate opened the employee lunged forward and to the left injuring causing pain in his left shoulder.

The employee was turning a chuck key with a T-Bar on a lathe in the machine shop. As the chuck and the chuck key moved downward his left hand ring finger was caught between the chuck key and the lathe table. The employee instinctively pulled his hand back, the chuck key caused a laceration which required 5 sutures.

The employee was originally injured on 2-11-12 while descending a ladder in a fluid bed dryer the employee tripped on the bottom rung and fell 12 to 18 inches backward. The original diagnosis was bruised ribs, 1 week later the employee, due to continued pain, went to the doctor again and was diagnosis with non-displaced fractured ribs 3,4,5 which then made this injury reportable.

Employee was cleaning a railcar when the car cleaner bar popped out of the hub and hit his right knee on the inside. No medical treatment was sought at the time of injury. On 12/21/11 the employee asked to seek medical treatment and received it. The employee was given restrictions and a required follow up appointment. This was reportable as of 12/21/11.

Injured employee was assisting pipefitter crew during pipe installation. Pipe was in a bind and employee attempted to push it free. Pipe released from bind pinching fifth digit on left hand between pipe and angle iron resulting in laceration and fracture. Employee received medical examination, sutures and finger splint.

Employee was carrying heavy tools and a chain hoist. When he stepped up on the pump platform, his left foot hooked the pull-eye. This caused employee to lose balance and twist, driving his left knee into the pump frame. Was driven to the doctor and then came back to work. No kneeling for two weeks. Accident was 6/1/11 but employee did not go to the doctor until 6/3/11.

EE was operating a HDPE fusion machine fusing two 16" HDPE pipes. While loading the machine, the section of pipe jammed on the saddle not allowing the pipe to feed properly. The ee attempted to reposition the pipe by hand leaving his left hand exposed in a pinch point. The pipe came loose and went forward pinching his #2 and #3 digits on his left hand.

The structural member was approximately 300lbs and when the ee pushed, he felt a pop in his neck. He returned to work where he continued for several additional minutes before his neck began to hurt. He then told site management.

IE was working on a pick up truck when he got a small metal sliver in his left index finger. He removed the sliver and continued to work. He developed granuloma at the spot where the sliver was removed. He went to the doctor a few days later and the doctor said that surgery was needed to fix the problem. He had surgery and began missing work on 09/27/2007.
