THE LADDER SLIPPED & EE FELL ~5'. HE WAS MOVING FROM ONE WORK LOCATION TO ANOTHER WHEN HE FELL. HE DID NOT HAVE HIS SAFETY ROPE TIED OFF DUE TO MOVING LOCATIONS. HE FELL ON HIS LEFT FOOT AND INJURED IT WHEN HE HIT THE FLOOR.

EE WAS MOVING FROM ONE WORK LOCATION TO ANOTHER. HE STEPPED ON A PIPE AND THE PIPE MOVED. AS HE FELL, HE GRABBED THE COLUMN WITH HIS HAND AND WHEN HE WEIGHT HIT, THE ELBOW AND ARM WAS SPRAINED AND STRAINED. HE DID NOT HAVE HIS SAFETY ROPE ATTACHED DUE TO CHQANGING WORK LOCATION.
