Employee was welding bars inside the trommel, using 2x6 boards and 3/4" plywood to sit on while welding. As the employee attempted to stand up, one of the 2x6 boards broke causing imbalance and resulting in a strained hamstring, and was giving an anti-inflammatory prescription. Employee was returned to full duty.

Employee using come alongs and chains to move curtain in Hazemag crusher. Chain broke and hit the employee in the hard hat. Causing the hard hat to give the employee a laceration on forehead and and safety glass causing a small cut on nose.

He was sent for evaluation and the physician recommended further testing to confirm if his condition was directly related to his work activity. Received confirmation from physician on 07/20/2011 that is directly related to his work activity.

While working with a torch to cut a steel pipe that was apaproximately 12" from the highwall and anchored at the top. The pipe moved away from the highwall approximately 20" and dislodged rock. Several rocks fell, one striking injury on the head knocking his hard hat off. A second rock fell striking him on the head, causing a laceration.

The employee was getting out a prybar out of the service truck and hit his rt thunb on the side of the door causing a tiny fracture. He went to get an x-ray and was determined positive and drainage of blood under nail plus immobilization of the thumb.

An employee was stripping a barge of the haul cables and the haul cable was caught on the outside of the timberhead causing tension. The employee went to release and the cable struck the employee in the nose/mouth causing a laceration and hair-line fracture to his nose. The employee was sent to the emergency room and conducted X-rays and an MRI.

THREE EMPLOYEES WERE REMOVING DUSTPIPE FROM THE PLANT. THE DUSTPIPE WAS SLID ALONG A HANDRAIL AND THEN DROPPED ONTO A WALKWAY. WHEN THE DUSTPIPE WAS DROPPED TO THE WALKWAY THE EMPLOYEE'S HAND WAS CAUGHT IN BETWEEN THE END OF THE DUSTPIPE AND A METAL AIR LINE.
