Boulder was lodged in Jaw Crusher of plant. Plant Operator looked into Jaw to get a better view of the obstruction when the boulder popped hard against an iron plate lining the jaw opening. Injured party was wearing proper eye protection, but what ended up being a sliver of metal went underneath bottom rim of eye protection and become lodged in left eye.

Employee was tightening a bolt on screen with a socket wrench. The Wrench slipped and slammed hand into the side of the screen. Employee seeked medical attention to ensure it was not broke.
