Essentially it came down to the operator of the portable band saw becoming complacent. The trigger was accidentally depressed with right hand while the left hand had came in contact with the blade of the band saw. It was the end of the shift and worker was cleaning up shop. The worker received a total of 6 stitches to the left index finger.

While compressor was warming up it appears the supply air valve was left open allowing air into the drill steel. The air pushed the water in the valve to a discharge hose causing the hose to whip & strike the employee in EE's left leg.

EE had almost completed mucking out the face when a rock (approx. 18" l.x 16" w. and 12" thick) fell out of the back. The rock then struck the boom of the loader which deflected it towards the operators cab. EE saw the rock coming towards him and raised his arm to protect himself. Rock struck the EE's arm causing a laceration.

Miner was operating air slusher when a jackleg drill fell over. Drill was propped up against the wall next to the slusher. Vibrations from the slusher caused the drill to fall over. Miner's thumb and palm were caught between the drill and slusher handle. Miner received a laceration to the left thumb and palm requiring stitches.

EE Fell of ladder landing on pallet of drill core with some landing on his chest . While getting up he reached up and knocked a bottle of HCL onto the floor, some splashing on his clothes. While pulling shirt off he got traces of acid on his face and nose and eyes and got burned. EE was transported to the hospital.

EMPLOYEE WAS INSTALLING GROUND SUPPORT (ROCK BOLTS). AT TIME OF ACCIDENT WAS DRILLING A HOLE INTO ROCK IN WHICH TO INSTALL A BOLT. A ROCK FROM THE BACK AND IN FRONT OF THE DRILL FELL ONTO A MUCK PILE AND FLIPPED OVER LANDING ON THE EMPLOYEES RIGHT FOOT.
EMPLOYEE WAS WALKING DOWN DRIFT. TRIPPED OVER ROCK. FELL FORWARD LANDING ON ROCK AND HITTING HIS CHEST ON THE ROCK.
EMPLOYEE WAS RIDING OUT OF MINE ON BACK OF MUCK TRAIN. WAS APPROXIMATELY 100' FROM THE PORTAL WHEN THE EMPLOYEES HEAD WAS STRUCK BY A LOW CAP(TIMBER SUPPORT). AS THE TRAIN WAS MOVING FORWARD EMPLOYEE WAS KNOCKED OFF AND LANDED ON THE GROUND.
CREW WAS HAULING 20"X10' VENT PIPE INTO MINE USING TIMBER TRUCKS AND ELECTRIC LOCI. MECHANIC WAS RIDING ON TIMBERTRUCK BEHIND TIMBERTRUCK WITH VENT PIPE ON IT. WHILE TRAVELLING INTO MINE A VENT PIPE HOOKED A TIMBER PUSHING THE VENT PIPE BACK HITTING THE MECHANIC ON HIS LOWER LEFT LEG. ACCIDENT IN MAIN HAULAGE DRIFT

Employee was traveling from a surface drill site back to the mine property when he made a sharp corner too fast. The UTV he was driving slid sideways on the road and turned onto it's side. The employee's left hand struck the ground or UTV fracturing his fith metacarpal in his hand.
