EE was clearing a jam from a conveyor #1 head box chute. As the jam was cleared the EE's left finger was wedged between the pry bar and a cross brace. The result was a fracture to the left ring finger.

The injured employee was helping replace a conveyor pulley. The pulley was moved into place with a crane, and was then suspended by a chain. As he attempted to remove the lifting slings, the pulley shifted and his arm was caught between the chain and the pulley.

The employee was cutting a bolt out of a bearing housing using a 10 foot long burning bar. As he withdrew the bar, flame blew out of the hole and burned his arms and face.

DURING PLANT OPERATIONS, BOLTS WORKED OUT OF THE FEEDER BOX UNDERNEATH THE BIN, CAUSING THE FEEDER BOX TO DROP INTO THE FEEDER. THREE EMPLOYEES ATTEMPTED TO PUT THE FEED BOX BACK INTO PLACE WHEN THE BOX DROPPED DOWN INTO THE FEEDER, PINCHING THE EMPLOYEE'S RIGHT THUMB, CAUSING HIM TO GET STITCHES IN HIS THUMB.

Water hose blew off the hose connection bushing. The employee put it back on and tightened it with a 6" crescent wrench. He turned the water back on and walked away. The hose blew off again and the water hit him in the back, knocking him into the feed box frame. He struck his left eyebrow on the frame and cut it.

INJ EE & ANOTHER EE WERE ATTEMPTING TO RETRACT THE LANDING GEAR OF FUEL TANK WHEN THE LEG GAVE LOOSE QUICKLY WITH NO TIME TO REACT. THESE LANDINGS ARE HEAVY LEGS THAT HAVE TO BE LIFTED MANUALLY AND FROM UNDER TANK. EE'S THIRD FINGER ON RIGHT HAND WAS INJURED RESULING IN LACERATION & FRACTURE
