The employee was using a large hammer and striking wrench to loosen the locking nut on a crusher mantle. The employee stopped to steady themselves and put a hand on the edge of the wrench. A burr had been raised on the wrench edge due to the hammer blows, which cut through the employee's glove and into their left palm.

The employee's left shoulder popped out of joint while climbing down into a jaw crusher to perform maintenance.

While changing V-belts on the #2 shaker screen, a come along was being used to lift the motor base. During this action, the tension spring rod broke, causing the spring to fall out and strike the employee on their right knee.

Employee was performing maintenance on a conveyor. Sunny, temps in the 90s, and high humidity. The employee had been taking breaks, and water and popsicles were available. EE reported feeling strange, lightheaded, experienced cramps in their arms, and vomited. Employee had been sick previous weekend, later confusion if issues were related to heat or previous illness.

While pulling on a piece of replacement rubber belt, the employee's foot was struck by the wooden piece connected to the rubber. The wood hit their foot, bruising it and caused them to step backwards awkwardly. Self-care was advised by care coordinator, but symptoms didn't resolve. On 4/19/24, employee was taken for additional treatment and received work restrictions.

EMPLOYEE WAS PICKING UP A 40 LB PIECE OF ROLLED-UP CONVEYOR BELT WHEN HE STRAINED HIS LOWER BACK. HE WAS ISSUED WORK RESTRICTIONS, COMPLETED PHYSICAL THERAPY, INCLUDING ELECTRICAL STIMULATION, AND RETURNED TO WORK FULL DUYT ON 9/8/03.
