Lafarge Building Materials, Inc. · Fall to the walkway or working surface

At 9am driller observed EE's knees buckling and going to the ground. EE insisted being fine just tired from lack of sleep. He finally agreed to be driven to hospital. He was admitted for heat exhaustion and severe dehydration. Later diagnosed with knee contusion and hairline ankle fracture from fall. He was released 8/16. His return to work is pending medical release.

Empolyee was performing cleanup in the plant near the secondary crusher when the crusher backed up causing material to overflow. The overflowing material struck the employee on the left side of his back above the belt line.

Employee was replacing bolts in the grizzley bars. He was removing rocks and layed his hand on the side of feeder when a rock fell onto his hand, cutting his left index finger on the inside.

Employee was exiting the 66 crusher platform and stepped on a golf ball size rock and turned his rt. ankle. Employee sought medical treatment and became recordable on 03/24/2006.

EE WAS ATTEMPTING TO REPAIR A HOLE IN A ROCK CHUTE. WHILE CONDUCTING THIS TASK, WELDING SPARKS CONTACTED THE EE'S LEFT HAND CAUSING HIM TO DROP THE PLATE, & PULL HIS LEFT ARM DOWN. THIS CAUSED THE EE TO STRAIN HIS LEFT SHOULDER.
