Employee used a wedge to relieve pressure in the primary jaw crusher in order to get a bolt out. When the employee was removing the wedge, the stored energy popped the wedge out, striking the employees right hand.

While removing accumulator on HP500, the accumulator fell unexpectedly. While attempting to catch the accumulator, ee's index finger became caught between the accumulator and side of the crusher mainframe. EE's finger was cut through gloves and required 7 stitches.

EE was removing zip ties from hydraulic hose with a pocket knife and cut their hand. EE was taken to the emergency room where they were treated with one suture and tetanus shot.

Hydraulic cylinder swung out, struck employee on left hand. It broke the knuckle of ring finger on left hand.

While installing a 2400R35 on Cat 775. EE was prying back the Rim flange with a 90 degree tire bar. The flange was rusted to the hub and EE was having a difficult time prying the flange loose. EE pushed with all of EE's might and strained EE's left shoulder.

Employee was cutting a card board drilling tube with a utility knife. The utility knife slipped and he cut his left leg right above the top of his boot. He was taken to the doctor and received tetanus shot and five sutures.

EE was mounting tire back on unit. EE used a push bar to set flange and lock ring. While putting the lock ring in place the push bar slipped and the tire flexed which struck the index finger on the left hand and the middle finger on the right hand.

It had been raining all day. At the time of the accident the rain was very heavy and oxygen bottle was slippery. The injury that resulted was a lacerated/smashed finger tip on right hand ring finger when attempting to load new oxygen bottle in mechanic's truck.

An employee with 35 years of experience, seven years at this company, was found to have a hearing loss through the annual audiometric testing. The employee was sent to the doctor whom confirmed that the employee was suffering from sensorineural hearing loss caused by a combination of noise exposure and Presbycusis (aging process).

MANAGER WAS ENTERING THE SCALE HOUSE OFFICE WHEN HE SLIPPED ON A WET AREA OF CERAMIC TILE. HE THEN FELL TO THE FLOOR ON HIS RIGHT KNEE AND RIGHT HAND. THIS CAUSED HIM TO INJURE HIS HIP AND LOW ER BACK.
