Maintenance employee had completed greasing of Bird Centrifuge and when he turned to step down to the floor, his foot slipped and he fell backwards. The ledge that he stepped on was wet from wash-up, causing him to slip.

EE was picking up a load of slurry clay: he had left his truck & was at the base of steps leading to loadout office: where he slipped. The area was wet there was some algae under the steps, but not in the walking area, grating was also in place to prevent slips. He reported that he had slipped. He said that he was not hurt, other than his rear. We learned 10/11/05 he is off work.

EE PRESSURE TESTED A TANK CAR & FOUND A FAULTY DISC ASSEMBLY; HE REMOVED THE OLD ASSEMBLY, BUT FAILED TO BLEED PRESSURE OFF THE CAR; THE RUPTURE DISC ASSEMBLY BLEW OFF, STRIKING HIM IN THE FACE.

EE USED A METAL POLE TO UNSTOP A RAILCAR AND PLACED THE POLE ON THE GROUND. HE THEN CLIMBED ON TOP OF THE RAILCAR TO BLOW IT OFF. WHEN HE CLIMBED DOWN THE LADDER, HE DID NOT LOOK WHERE HE WAS STEPPING. HE STEPPED ON THE METAL POLE HE HAD LEFT ON THE GROUND AND SPRAINED HIS FOOT. EE WENT TO THE EMERGENCY ROOM THAT NIGHT AFTER LEAVING WORK.
