Maintenance replacing the trunnion/bearing rollers on the rotary screen. While attempting to remove the strap holding the roller/bearing, the weight shifted and caused the roller and bearing to shift and struck the Techs right hand causing a laceration to EE's (R) index finger, requiring 7 stitches.

Maintenance Mechanic working on a bearing to a fan completed the repairs and was re-installing a pulley guard, the guard got stuck and as the Mechanic pulley on the guard to free it to complete the installation EE's (R) ring finger and pinky finger got pinched between the guard and the fan. Causing a fracture to the tip of EE's ring finger.

Operator heard an unoccupied CAT D6T dozer sound as if it was trying to start, detected smoke around the starter. After brief observation, any smoldering appeared to stop. After 20 min, flames were noticed and deemed unsafe to contain. Fire was monitored from a safe distance and secured the entrance to the pit

The lead mechanic/electrician reached out to pull electrical wire to place on a pole and heard right shoulder pop. First visit to a medical doctor was Monday 06/07/2021. During this exam the mechanic was placed on restricted duties.

While conducting a spot check for oversize particles in the glass sand screen room, the employee struck left shoulder on a diagonal stationary chute while crossing under it. Danger "Low Head Clearance" signs were in position on both sides of the VS-03,05, 06 chute. Employee first day missed from work was 05/17/2021.

A pipe line had been taken apart, flushed out, and bolted back together. Pump PU-03 was started up to flush the pipe line and water hammer occurred. It ruptured the poly pipe line, the pipe struck one mechanic on the back of EE's right lower leg. The injured employee was medically evaluated and placed on temporary restricted duties.

On 01/24/2020, the mine was informed that its workers compensation insurance deemed and approved a loader operator's right elbow pain as work-related. The loader operator informed the mine operator of the concern on 01/06/2020. Medical treatment was obtained.

On May 03, 2019, former employee reported to the Company that EE had surgery on right foot that EE sprained on EE's last day of employment with Covia. The former employee had reported stepping from a walkway to the ground and felt pain in the right foot. EE was advised to go to the doctor and inform Covia of medical treatment.

On 8/6/19, the employee needed left shoulder surgery was deemed as work related by Workers Comp Ins. On 03/27/19, the employee reported pulling/straining EE's left shoulder on 03/25/19 when climbing a ladder seven time to adjust chemical pumps instead of using stairway. No medical attention needed or requested at the time. Date of first day of lost Time 08/19/2019 due to surgery.

While removing a 2" diameter pipe using a 18" pipe wrench, a 2" connector broke. The pipe which was under pressure broke loose and struck EE on EE's left eye brow causing a laceration. The employee was examined, treated, and eight sutures placed above EE's left eye.

Employee was operating excavator in the mine (pit). The operator crossed an established berm and was attempting to move clay/kaolinite from the high wall face to access better sand. As he was backing up, he saw the kaolinite caving off. He did not have time to prevent the boulder from striking the excavator cab. The employee was pended in the damaged cab with sand up to both knees.

Injured was handed a 4X4' section of liner from the flat plant attritioner bank (D) and lost his balance. He fell against the attritioner bank (C) which had bolt ends protruding from its side wall. A bolt end lacerated the injureds left forearm requiring 11 sutures.

Employee was changing a metal flange at cyclone number 15 in the flot plant. He removed his gloves to place nuts on related bolts, then lifted the flange before placing gloves back on his hands. The flange had a sharp wear point on the inside which lacerated his right middle finger.

An employee & co-worker were traveling on plant road at less that 15 mph. The driver became unconscious. The vehicle veered to the right and as the vehicle started to strike a berm the passenger placed the vehicle in park. Seat belts were being worn. Incident/Illness not work related. Ambulance transported employee to hospital as a precautionary measure.

Injured went to tighten packing on PU13 at ground level in the plant. He selected a route that involved climbing over 2 pipes. On the second pipe he crossed, he stated he slipped and fell against his right side of back and ribs. Injured reported it the next day.

EE traveled provided steps and fixed ladder to change valves to divert sand flow from new to old tower. EE took a non access short cut by climbing under handrail to return to main cross over walking from the fine tower to the main wet plant. This short cut would have required walking on a pipe. EE slipped and fell approx. 40ft.

HE CAUGHT ALL HIS WEIGHT ON HIS LEFT HAND AND SPRAINED HIS LEFT WRIST (THE WRIST HAD BEEN INJURED PREVIOUSLY IN A NON WORK-RELATED ACCIDENT).

THE INJURED WAS PLACING A WIRE CABLE SLING ON THE LOADER FRAME AFTER PULLING OUT A STUCK CRANE TRUCK. THE INJURED LIFTED THE CABLE BY ITS EYE WHICH HAD A BURR STICKING UP FROM IT. THE BURR ON THE EYE OF THE SLING LACERATED THE INJURED'S LEFT INDEX FINGER. THE INJURED WAS NOT WEARING GLOVES.

A 6" C VIS GRIP CLAMP WAS BEING USED BY THE MAINTENANCE TEAM TO ASSIST IN THE REMOVAL OF THE GLASS SAND ELECTOR CONVEYOR BELT FRM THE BOTTOM OF THE SILO ELEVATOR. THE VISE CLAMP WAS ATTACH TO THE CONVEYOR INCLUDING A ROPE TO PULL THE BELT FROM THE ELEVATOR. THE VISA CLAMP PULLED LOOSE AND STUCK EE S RIGHT HAND, INJURING HIS FINGERS.
