After haul truck operator backed up to primary crusher, crusher operator saw small fire near the passenger side front tire which grew quickly. EE called for the driver to evacuate, and both left the area. Driver reported no prior issues. Due to extensive damage, source/cause of fire is unknown. Believed to have started either in engine compartment or near front passenger tire.

Employee was retrieving scrap parts from the bed of a service truck. When the employee bent to pick up a part weighing approximately 10 pounds, suffered a lower back strain.

Employee was in the process of lighting a cutting torch during routine maintenance activities. Seconds after lighting the torch, there was a loud noise nearby on the coiled hose reel. A rupture in the oxygen hose produced the noise, and due to proximity, resulted in the employee having ringing in the ear and subsequent temporary disorientation.

Employee had a first aid case in Feb. 2022. Reported further aggravation of this and a pre-existing condition on May 17 2022. Medical evaluations in June 2022 revealed possible aggravation to pre-existing condition in knee. Surgery required and performed on 7/11/22, releasing miner to work on 7/12/22 with restrictions.

The employee was cutting a cardboard tube used to keep debris out of the blast hole, but the tube was difficult to cut and slipping. Employee was using a safety knife with debris in it and it slipped off the tube, didn't retract, and cut the employee's right forearm. Employee needed 8-10 stitches.

Employee was washing with a 1 inch water hose equipped with a ball valve fitting at the end. During the process of cleaning, EE had laid the hose on the catwalk. Pulling the hose, the ball valve caught on a toe plate, turned the water on, causing the hose to whip under pressure, striking the employee on the left eyebrow.

While performing repairs to a conveyor drive coupling, the injured employee stepped onto a motor base, approximately 8-10 inches in height. Foot slipped off the ledge of the base, causing right ankle to twist under body weight, resulting in a fracture to the right fibula.

Employee was exiting parked personal vehicle in the employee parking lot when employee slipped on ice. The employee suffered a fracture to leg when employee fell on the ice. Employee had not yet reported for work.

Employee sustained an injury when attempting to loosen material in a chute with a pry bar. The employee reported the event, but requested no medical attention until 1/10/2019. Clinic visits and follow-ups with specialists revealed an injury requiring surgery. Surgery performed on 3/5/2019. Employee released 3/6/2019 with restrictions.

Employee stated he was stood on the side of the crusher and the canopy over the redundant air compressor which is 23 inches above the floor tightening a bolt. When completed the employee hopped off the 23 high ledge onto the broken 3 bolt which had been left in the work area on the floor, twisting his right ankle. An x-ray showed a fracture of the right ankle.

Drill techs shirt sleeve got caught between rotating rod and chain sling being used to load rod on the drill. Individual sustained a broken elbow, cracked rib, cut on his forehead and abrasions to his chest, arm, and neck from his clothing.

EE using a sledge hammer to remove pins from a cone crusher when he exper. pain in his shoulder. Injury not reported until late June. Investigation was made to determine if the claim was work related. The decision was made to honor the claim as work related. All restricted days from treatment will be included on RTW filing.

Employee was changing teeth on a 990-H loader and struck one of the teeth with a sledge hammer in an attempt to loosen the tooth. This caused a piece of metal to dislodge from the tooth striking the employee on the inside of his left leg approximately 4 inches above his knee. The metal penetrated his leg lacerating an artery.

Employee lifted a section of 1 inch diameter metal pipe and slipped, which resulted in him injuring his lower back. He was taken to the doctor and diagnosed with a back strain. Treatment included the doctor prescribing medication which made this a reportable injury.

Employee was conducting an inspection of the wash screen prior to starting up the plant. While walking down the top deck of the inclined screen he slipped and fell, striking the side of his body against a section of angle iron in the discharge chute. Injuries sustained included cracked ribs and an abrasion to the right wrist.

Employee was using a hammer to install a steel band to secure a hose to the heat exchanger. While attempting to tighten the band with the hammer the employee struck his left index finger. This resulted in a laceration that required three stitches to close.
