Employee was operating front end loader while digging in the face of a sand pit. While digging, the loader struck a large rock causing the employee's head to lurch forward. The employee did not seek medical care until 10/18/10. He was diagnosed with a neck strain and put on restricted duty. He was returned to full duty on 10/25/10.

EMPLOYEE WAS CLEANING AROUND THE SCALES. THE SCALE SURFACE WAS REMOVED FOR REPAIRS. THE EMPLOYEE WAS WALKING ALONG A PIECE OF I-BEAM, APPROXIMATELY THREE FEET OFF THE GROUND. WHEN THE EMPLOYEE SLIPPED OFF THE BEAM AND CUT HIS RIGHT LOWER LEG.
