Driver was removing a lump of material from tailgate of dump truck. Driver instructed a 2nd driver to close the latch. Driver#1 placed hand on the pin where the clasp locks the tailgate. Middle finger on right hand was pinched inside the clasp.

EMPLOYEE WAS CLEANING OUT A LARGE SCALE SYSTEM. HE WAS LIFTING BUCKETS OF DIRT AND MUD OUT OF THE SCALE PIT. THIS LIFTING MOVEMENT CAUSED A HERNIA. EMPLOYEE WAS OPERATED ON JULY 6TH, 2001.
