Employee installing new belt onto blunger drive when finger was pinched between pulley and belt.

Forklift brushed employee's ankle.

Employee was standing on a ladder installing an access panel weighing approx. 10 pounds. The door was not secured and fell approx. 12 inches striking their right shoulder.

The employee became overheated while replacing load cells for a scale inside of a building. The ambient temperature was well over 100 degrees where the employee was working. The employee was short of breath and their arms were tingling.

Employee dropped roll of stretch wrap on to unprotected area of boot causing contusion on right foot.

Employee was hanging bulk bags when experienced pain in right arm. First aid was administered and employee finished shift. Employee was still experiencing pain days later and was sent to see Doctor. Placed on restricted duty and given medication for tenosynovitis.

The elevator an EE was using malfunctioned and stopped approximately 6 ft from the ground level. The EE was stranded in the elevator for approximately 2 hrs. after which, they decided to attempt to exit. As the EE exited the elevator, they fell to the grating on the ground level, an approximate 6 ft fall, striking their side and back. The EE fractured 2 ribs.

A mechanic injured their right thumb when a motor fell on it.

Employee Struck by Pallet with Product transported by Forklift. Employee's left ankle was struck causing sprain that required medical attention. Ankle was put in a boot and Employee was put on Restricted duty with medical follow up in 6-8 days.

A Lab Technician experienced a skin injury when EE spilled Nitric acid from a test tube while performing standard lab test under a lab hood. The spilled liquid contacted EE's neck and arm creating small first degree chemical burns on skin. The Doctor prescribed antibiotics and prescription burn cream. The employee returned without restrictions the same day.

Mechanic was using 2 lb hammer to drive pin into valve while performing maintenance activities. As the hammer struck the pin, the employees 5th finger (pinky) struck metal valve mashing finger between hammer handle and valve. The employee received 7 stitches in 5th finger. Restricted duty only, no lost days.

Injured employee was helping to demolish and old fence when a piece of angle iron fell striking employee on the upper right arm. The arm was not broken but there was a soft tissue injury. The employee has returned to work on restricted duty as of 10/28/2016.

Mechanic was exiting a dryer control room. The mechanic placed his left hand on the door to control the door. Fifth finger (pinky) was pinched between door and frame causing laceration above finger nail bed. Employee received stitches and prescription for antibiotics and pain. Returned to work with restricted duty.

Original incident occurred on 8/14/2012, when employee bumped head on a filter trough and was treated for first aid, as well as a personal preexisting condition. On 3/18/2013, the site Medical and EHS were notified that surgery had been performed on 3/6/2013 and was being covered by WC.

Turning trailer landing gear handle when it flipped back striking left hand.

MidGA - Gordon. A Gordon employee received an injury when opening the #7 filter vacuum pump valve. The handle broke lacerating his left index finger through leather work gloves worn as proper PPE for the task. The employee was transported to the Medical Center in Macon where he was treated and received four stitches along with a prescription for antibiotics.

Employee felt a pop in his shoulder while throwing trash into a dumpster. Employee did not report incident until 9/10/12. Initially treated as first Aid by Nurse. On 9/21 asked to see Doctor, who placed employee on lifting restrictions. Reclassified as Restricted day case on 9/21.

At 1:40 PM. mechanic ascending ladder at 4 bagging struck top of knee on rail. Reported at 2:30 PM. to supervisor and complained of tightness in right knee.

A Translink operator was pulling a filled 50lb bag from the packer spout to place on the conveying system when he lost his grip on the bag. In an attempt to prevent the bag from hitting the floor, the operator tried to regain control of the bag and felt a pain in his lower back. Late reporting of injury due to causation investigation. Injury cause was determined on 11/01/10.

Mechanic opened the right side of a double door to the atomizer feed pump pit and stepped inside. Several seconds later a short section of purlin with attached metal siding broke loose and fell striking the employee in the back of his hard hat and upper right back. This caused the employee to move forward at which time his right lower leg came in contact with a platform.

Mechanic was performing maintenance work on a bauer mill. A bolt was stuck in the bottom and when he tried to clear it, the rotator assembly moved and piched the distal end of his left little finger between the assembly and the bauer wall. Employee sustained a laceration to the distal end of his left pinky finger.

Process Specialist suffered from heat stress while working in the calciner area. Employee was in the maintenance area observing maintenance activities for approximately fifteen minutes before exiting the area and became ill upon returning to his office.

A mechanic was lying down inside the 1st stage blunger tank at 1-C Cowles attempting to remove the bottom blade. After an unsuccessful attempt at removing the blade, the mechanic lost his balance causing his left hand to come intact with the blade. He sustained a laceration across the palm of his left hand.

EE alleges cumulative trauma to his right ring, middle, index and thumb as a result of performing the essential functions of his job which includes manually removing 50 lb bags of kaolin from a spout and placing them onto a conveyor belt. States the onset of his finger complaints was approximately 2 weeks prior to reporting date. Date of doctor visit was 11/1/06.

The employee was returning to his golf cart from the #4 Bulk Loading area when he slipped and fell. Apparently, in an attempt to brace or recover from the slip, the employee broke his right ankle. The area where the fall occurred is an incline surface. There was wet kaolin in the area where the fall took place.

Employee attempted to lift a pipe threader and load it without the forklift and strained his back.

Employee was checking the belt number on the drive belts and the conveyor started in motion. Employee's left hand went through the nip created by the drive belt and the sheave, resulting in laceration to left index finger and compound fracture to left middle finger.

Employee states while working on the big bagger, the neck of the bag came off the spout causing kaolin to spill on top of the one ton bag. Employee states he felt a pull in his back while bending over to clean up the kaolin spill.

EE WAS TIGHTING BOLTS IN THE BOTTOM BEARING OF A BAUER MILL WHEN HE FELT A PULL IN RIGHT GROIN AREA AND FELT A SMALL KNOT IN THAT AREA. EE SOUGHT MEDICAL TREATMENT ON 12/01/2004 AND WAS RELEASED TO REGULAR WORK.

REPORTABLE HEARING LOSS LEFT EAR. TESTED 7/1/04 CONFIRMED BY DR DOCUMENTATION 2/7/05

EE WAS REMOVING A FEED TUBE FROM #1 MAGNET WHILE IN A SEATED POSITION AND USING A 24" PIPE WRENCH. AS HE PULLED HE FELT A PAIN IN HIS BACK AND DOWN HIS RIGHT LEG. WORK COMP PHYSICIAN DIAGNOSED HIM AS HAVING MUSCLE STRAIN TO LOWER BACK. HE WAS GIVEN AN INJECTION, 2 PRESCRIPTIONSAND PLACED ON RESTRICTION AND NO LIFTING OVER 40 LBS.

EE'S RIGHT RING FINGER WAS LACERATED WHEN THE BAR HE WAS USING TO CLEAN LUMPS OUT OF A BAUER MADE CONTACT WITH A COVER. HE WAS TRANSPORTED TO ER BY CO-WORKER & HAD SURGICAL REPAIR ON 8/29/02. FINGER WAS FRACTURED IN 2 PLACES.

PROCESS ENGINEER WAS WALKING PAST A HANDRAIL WHEN HIS RIGHT ARM CONTACTED A PIECE OF CORRUGATED METAL AROUND THE RAIL RESULTING IN A 2 1/2"LACERATION TO HIS RIGHT FOREARM. HE WAS SEEN BY A PAN EL DR. WHERE HE RECEIVED 6 STITCHES & WAS RELEASED TO FULL DUTY.

EMPLOYEE LAID HIS IMPACT WRENCH WHICH HE WAS PLANNING TO USE TO TIGHTEN BOLTS ON THE LID ONE HE CLOSED THE LID ON THE TANK CAR. HOWEVER, WHEN HE CLOSED THE LID WITH HIS LEFT HAND, HIS LEFT LIT TLE FINGER WAS CAUGHT BETWEEN THE LID AND IMPACT WRENCH. HE SUSTAINED A LACERATION AND FRACTURE OF HIS FINGER.

WORKER HAD MOVED TIN THAT HAD BLOWN FROM THE TOPOF #2 FELTER BLDG TO THE GROUND DURING THE NIGHT, WHEN WORKER PICKED UP THE PIECE OF TIN, THE WIND CAUGHT THE TIN AND FOLDED IT OVER, STRIKING E MPLOYEE'S RIGHT ARM AND LACERATED IT. EMPLOYEE WAS WEARING GLOVES.

EE SLIPPED & FELL FROM LOADING DECK. FELL APPROX41/2 FT

EE WAS MOVING A 4" HOSE TO MAKE A HOOK UP TO FILTER. STRAIN TO LOWER BACK RESULTED.

EMPLOYEE WAS CLOSING SPRING LOADED DOOR ON HOOPER CAR WHEN HE FELT PAIN IN LOWER BACK AND GROIN AREA.

EMPLOYEE WAS HELPING TO RUN PAPER STACKS ON CALENDER STACK ROLLER. RIGHT HAND WAS CAUGHT IN CALENDER STACK ROLLER WHILE TRYING TO TAKE PAPER OFFROLLER.

EE WAS REMOVING FILLE PAPER BAG FROM BAGGER SPOUT WHEN HE EXPERIENCED PAIN IN BACK. BACK STRAIN RESULTED.
