Maintenance employee slipped while moving replacement parts for truck slicer. No serious injury or medical attention needed. Employee continued working scheduled shifts until 4/23/20 when EE sought medical attention.

A commercial company truck driver was putting pressure hose back into place when the driver slipped off of truck and fell to the ground. Contributing conditions include: personal health issues/restrictions, and procedural error. They are, again, an over the road, commercial trucking company - this wasn't a man at our facility and they do not have a contractor ID.

EE was changing a silo line. While lifting the line, he felt pain in his shoulder. After weeks of therapy, a MRI on his shoulder on 3/6/15 revealed a "SLAP Tear"

EE was cleaning clogged kaolin out the #6 roller mill feed bin. As he was removing the clay from the interior wall, he mashed his finger against the door of the feed hopper. This resulted in a laceration and a broken finger tip on his left ring finger.

EE was removing drive belts from #6 roller mill fan motor sheave. A pipe wrench was being used to turn the motor sheave to remove the old drive belts. As he was turning the sheave with the pipe wrench, his finger was mashed between the pipe wrench and the bottom section of the guard for the fan drive.

Employee was opening the back (swing) gate for the crude delivery truck to exit the plant. He was holding the gate and walking to the "open" locking mechanism, he did not realize his hand was lined up with the locking mechanism. As he engaged the gate with the locking mechanism, his hand struck the locking mechanism.

Employee had cut off the air to the bagger and locked it out before he began to tighten a plate on it. While tightening the plate, he had his finger in the pinching mechanism and pushed the button to activate it at the same time. The pinching mechanism cut the tip off his middle finger on his right hand (flesh only - no bone involved).

A guard on the Caterpillar 938G right rear brake light guard had been bent. The mill operator was trying to straighten the guard when a bolt broke allowing the guard to come off and hit him in the chin. He was taken to the emergency room where he recieved three stitches in his chin. He was treated and released to full duty with no restrictions by the attending physician.

WHILE PREPARING TO THROW A SWITCH, EE NOTICED A STRING OF FOUR RAIL CARS MOVING INDEPENDENTLY DOWN THE TRACK. IN AN ATTEMPT TO APPLY THE RAIL CAR BRAKES HE SLIPPED AND FELL CAUSING HIS LEFT FO OT TO BE CONTACTED BY ONE OF THE RAIL CAR WHEELS.

A LIGHT RAIN HAD FALLEN DURING THE NIGHT SHIFT. AT THE END OF THE SHIFT EE WALKED ACROSS A CONCRETE PAD AND SLIPPED AND LANDED ON HIS BACK. HE COMPLAINS OF BACK PAINS. DIAGNOSIS- ACUTE LUMBAR STRAIN, ACUTE CERVICAL STRAIN.

EMPLOYEE WAS PLACING A PALLET IN PLACE TO STACK BAGS. AS HE WAS DOING THIS A 50 LB BAG FELL FROMTHE ROLLERS TO THE FLOOR, WHEN HE PICKED UP THE BAG, HE FELT SOMETHING POP IN HIS BACK. THE E.R. DOC SAID HE HAD MUSCLE SPASMS. THE DR GAVE HIM PRESCRIPTION FOR PAIN AND INSTRUCTED HIM NOT TO LIFT BEN SCOOP FOR ONE DAY.
