After removing a bearing housing, Miner 1 used a grinder with a cutting wheel to split bearing sleeve. Miner 2 took a hammer and hit the sleeve to knock it off. A piece of metal chipped off and struck Miner 1 on the right side of EE's abdomen

EMPLOYEE WAS HOLDING ROPE LOOPED IN HIS HAND TO CUT WITH UTILITY KNIFE. WHEN EMPLOYEE TRIED TO CUT THE ROPE THE KNIFE SLIPPED OFF THE Rope, came TOWARDS THE EMPLOYEE, AND LACERATED HIS LEFT ARM. THE LACERATION REQUIRED 7 STITCHES.

Employee was assisting with loading a hammer bit into the bucket of a backhoe. The bit was being pushed forward and it shifted and grazed the employees knee. It caused a laceration just above the right knee. The laceration required six stitches to close the wound.

EE and technician were filling hydraulic using lube truck (air operated pump). Handle was still on loader platform when end of control valve blew off. Handel blew across platform and into ground. Hose end hit mechanic on right foot ankle.

EE cut off his right middle finger while working on the hp-500 bowl. Crew was changing out the wear liners in the bowl of the HP-500. Nuts were placed under the bottom of the new liner to give the bowl clearance off the floor to allow adjustments. Grease is used to seal leaks. The grease created suction. When it released, the bowl dropped & amputated his finger.

AFTER EMPLOYEE THROWED SWITCH HANDLE ON SWITCH, HE WAS GOING TO STEP ON TOP OF SWITCH TO MAKE SURE THE SWITCH LOCKED, WHEN HE STEPPED ON TOP OF SWITCH HANDLE HIS RIGHT FOOT SLIPPED DUE TO MUD, HIS LEG STRIKING ON THE RIGHT SHIN AREA, WHICH AT THE TIME WAS THOUGHT TO BE A BRUISE.
