Employee was lifting a 992G loader shank into the loader bucket. The employee went to release the shank and felt a pop in lower back.

Employee lost footing while tramming manlift causing EE's foot to come off safety deadman switch. The machine stopped abruptly causing the employee to fall and striking EE's right shin on the kick rail.

Employee struck EE's left lower back on a conveyor support frame. Employee received bruising on EE's back.

As technician was torqueing wheel fasteners on the RRO of a 777 haul truck, the torque wrench shifted causing the wrench to crush the technicians left hand between the wrench and the wheel hub.

As employee was loading a jack plate back onto truck it slipped out of employee's hands smashing the middle finger on right hand

An employee was injured while using a torch to cut the nut off the back of a sand screw flight. The hot nut burned a hole through employee's pant leg which then allowed the nut to fall down into employee's boot causing a third degree burn to the right foot.

The employee was attempting to throw a flop gate, when a piece of metal fell from the chute striking the employee in the left forearm causing a laceration.

EE was removing old lip shroud from loader bucket using a pneumatic wrench. While holding the worn bolt head with gloved hand the bolt spun causing laceration to left index finger.

Slipped off the bottom step on the truck, fell into a hole at the shot, fractured right fibula.

Employee was cutting out a damaged profile on the screen deck with a torch. The employee lost his balance and fell onto the piece he cut off.

EMPLOYEE WAS STEMMING HOLES WITH A 5 GALLON BUCKET AND SPRAINED HIS BACK LIFTING THE BUCKET.

EE WAS CHANGING THE ENGINE BELT TENSIONER ON THE KOMATSU PIT LOADER WHEN INJURED.THE EE WAS PRYING THE TENSIONER BACK TO PUT THE BELT BACK ON, THE PRY BAR SLIPPED OFF AND THE EE'S HAND WAS PIN CHED BETWEEN THE TENSIONER PULLEY & FAN GUARD.

COOLING CONTAINER WAS NOT FLUSHED OR EMPTIED BEFORE PUTTING ON CRANE. THEN COOLING FLUID IGNITED AND STARTED TO BURN. AFTER USING TWO EXTINGUISHERS, FLAME STILL WAS BURNING. FINALLY IT WAS PUT OUT WITH DIRT AND MUD. BEFORE DOING SO, EE BREATHED SMOKE AND TOXIC FUMES IN HER LUNGS. DUE TO SMOKE INHALATION, THROAT AND LUNGS WERE BURNT. SHE WAS HOSPITALIZED.

EE OPENED DOOR TO GET OUT OF TRUCK WHICH WAS ON INCLINE. DOOR SLAMMED BACK ON RIGHT 5TH FINGER CAUSING LACERATION AND FRACTURE.

EE WAS DESCENDING LADDER ON HAUL TRUCK. HE LOST HIS BALANCE AND FELL.

WHILE EXITING DOZER D-30, OPERATOR SLIPPED AND FELL STRAINING HIS LEFT SHOULDER.

OPERATING OFF ROAD TRUCK WITH DEFECTIVE SHOCKS, HIT BUMP IN HARD ROAD AND INJURED BACK SOMEHOW.

EE STATES: I WAS PUTTING A FUNNEL ON CHUTE AND IT SPRANG LOOSE AND HIT MY FRONT TOOTH.
