Employee was inspecting under a transfer chute trying to align the chute with a box when EE was struck on the side of the head by a rock. The rock was resting on a beam directly above where the employee was positioned for the task.

Employee was exiting a Cat D-8 dozer when EE missed the bottom step and fell approximately 2 ft. to the ground. EE sustained a sprain to the rt. wrist. Possible unspecified fracture. Diagnosed on 8/24 as a fracture to the wrist requiring a cast.

The employee was operating an articulated truck. Employee stated employee fell asleep. The tire tracks indicate the truck rode up the left side embankment causing the cab to turn over on the right side. Employee received a concussion without loss of consciousness and a laceration to the right ear without foreign body.

Employee was exiting the Volvo Excavator while carrying the DEF fluid after inserting DEF fluid into the machine, while stepping down onto the foot step the employee foot slipped off of the pivoting step causing the employees body to twist resulting in shoulder pain. This incident was designated reportable based on number of physical therapy visits.

Employee was tightening the cheek plate bolts on the Primary Jaw crusher with a slug wrench and a 4lb hammer. Employee struck the frame of the crusher causing the hammer to glance off of the frame striking his left hand. Employee was wearing proper PPE (gloves and safety glasses)

At the beginning of the shift the employee was fueling his haul truck. After fueling, he got back in his truck and started to the pit. After wiping his face and eyes with his hand, he felt something in his eye. After reporting this to his supervisor, he washed his eyes out. This effort did not improve the situation, so he was taken to the company doctor for treatment.

Employee was pulling on a conveyor belt when he experienced pain in his right shoulder. He continued with his normal job duties without any lost time. After continuing pain, an MRI revealed that the employee suffered a muscle tear that will need surgical repair. The employer became aware of these results on 10/3/05, which at that time this injury became reportable to MSHA.
