EE was using a lever chain hoist to remove a large shaft. The welded eye failed and the broken piece struck the EE in the hand resulting in a laceration that required stitches.

Operator had climbed into the seat of the loader and went to shut the door. As EE pulled hard on the handle, EE experienced a sharp pain from the left elbow to the hand. Operator was swapping loaders with another employee.

THE HOIST CABLE BROKE ON THE DRAGLINE AND CAME DOWN THE BOOM. THE END OF THE CABLE FLIPPED INTO THE FRONT WINDOW OPENING WHERE HE WAS STANDING THE CABLE HIT HIM ON THE RIGHT ARM RESULTING IN A COMPOUND FRACTURE.
