Employee's hand was holding a liner clear of the bolt, the bolt caused the liner to jump causing employee's hand to move towards the spinning bolt which cut through EE's leather glove and into the base of EE's left palm.

At 3:30 am Shipping Attendant was monitoring the bagging operation when the employee heard an air leak. EE found a broken hose inside the applicator control cabinet. After isolating the source of air and attempting to repair ruptured hose, the knife EE was using slipped and cut EE's left ring finger causing a laceration. Stitches were required for medical treatment.

Process Attendant was checking the rejects magnet, the PA observed a piece of metal stuck to the magnet. When the PA had the piece of steel outside the plane of the door, PA attempted to remove the steel from the doorway when a rock came in contact with the piece of steel debris and PA's right thumb.

The PA took the buggy to an area where they could put water in radiator, they did not allow vehicle to cool down before opening radiator cap. PA had hot fluid spray on their hand and face causing 2nd deg burns to face.

Employee was climbing a staircase in the finish mill building when they missed the last step and fell onto left knee. Employee was taken to a local clinic for further evaluation. Clinic provided employee a knee brace and EE was told to not climb stairs or kneel. Clinic also advised employee not to stand for prolonged periods of time and to rest knee every two (2) hours.

Elbow pain continued to worsen until medical treatment was advised on 08/28/2020, at which time medical professional diagnosed lateral epicondylitis (tennis elbow. Employee was given tennis elbow brace by the doctor.

Employee was attempting to unplug a chute. EE opened a port on the chute, and shortly after the chute became positive and blew dust into EE's face and eyes. Flushed eyes immediately at the facility and then transported employee to medical facility for further evaluation. Facility continued to flush eyes and had an ophthalmologist complete an evaluation.

Flushed eyes continuously for 30 minutes. Followed up a few hours later and injury had not improved; so sent employee to receive medical evaluation.

A maintenance intern was observing mechanics replace pans on a conveyor when a co-worker pinched the intern in jest, startling the intern to the point where the intern's left knee buckled and the intern fell to the ground. The fall aggravated a previous knee injury to the point where medical attention was required.

Employee could not get EE's hand out of the way in time before panel smashed it. This resulted in a pinch point of EE's left index finger. Open laceration to the tip of the finger and nail bed being torn off.

The MT was using a 3/4" pneumatic impact wrench to drive a 1-1/2" socket for removal of flange bolts when the socket shattered. The MT could not locate a rated socket so made the decision to use a socket designed for hand use, resulting in the failure. The employee received and small laceration to EE's upper lip requiring 2 stitches.

Initial medical examination was negative for hernia, employee is continuing treatment and the final diagnosis is uncertain at this point. EE was holding the hand rail or the platform when the platform moved pinching the ring finger on his right hand between the hand rail and a diagonal brace resulting in laceration and broken bone of his right ring finger at the first joint.

EE was working in the Raw mill with over head cutting taking place. EE went to put on a pair of gloves to remove some metal from the area that had been cut away from the job taking place over head. When he put on the right hand glove he felt something like he had been burnt by a hot ember. But it was a brown recluse spider in his glove that had bitten him.
