Newberry Cement Plant Metal/Non-Metal

Employee grabbed a master link that was set on flange above EE's head and pulled cement dust into EE's left eye causing a chemical burn to the cornea.

Team member was replacing a 480V breaker inside switchgear with proper lockout. During this process, their screwdriver contacted a grounded surface causing an arc flash causing burns to face, arms, eyes and lungs.

Employee was restacking bags of cement onto a pallet when they felt a pop in their lower back.

Employee was washing the calciner area with a straight tip lance in the upward position. A piece of build up dislodged and hit the lance, jarring it and pushing it backwards into EE's left shoulder area. Causing a contusion.

Employee had right thumb caught between a handrail and the handle of a manual pallet jack.

Employees was adjusting blades on sepol fan in Finish Mill 1. The team was using an impact wrench to loosen set bolts causing dust to fall from above where the team was working. Dust fell between the employees face and safety glasses.

Employee was picking up a sheet of plywood when EE felt a pop in EE's right elbow. Initial diagnosis is a rupture of the right distal bicep tendon.

Maintenance Tech was relocating empty compressed gas cylinders when the Tech hit self in face with cap of bottle. Tech was pulling bottle to the edge of secure rack to roll cylinder, when pulled on cap to move, the cap came loose and hit left portion of safety glasses causing laceration to eyebrow

While using a jackhammer to clear material from a chute the bit became stuck. The employee pulled a muscle/tendon while trying to dislodge the jackhammer bit.

Failure to maintain 3 points of contact while exiting the truck.

Employee's hand was holding a liner clear of the bolt, the bolt caused the liner to jump causing employee's hand to move towards the spinning bolt which cut through EE's leather glove and into the base of EE's left palm.

At 3:30 am Shipping Attendant was monitoring the bagging operation when the employee heard an air leak. EE found a broken hose inside the applicator control cabinet. After isolating the source of air and attempting to repair ruptured hose, the knife EE was using slipped and cut EE's left ring finger causing a laceration. Stitches were required for medical treatment.

Process Attendant was checking the rejects magnet, the PA observed a piece of metal stuck to the magnet. When the PA had the piece of steel outside the plane of the door, PA attempted to remove the steel from the doorway when a rock came in contact with the piece of steel debris and PA's right thumb.

The worker was struck in the shoulder with a 150-200lb bearing that was suspended from a nylon strap that was holding the load. The strap broke and the load fell & bounced approx. 18-20 ft and traveled approx. 11-15 ft before striking worker. The work above was being conducted by another contractor PAR

Production Assistant was operating a buggy that was indicating is was overheating. The PA took the buggy to an area where they could put water in radiator, they did not allow vehicle to cool down before opening radiator cap. PA had hot fluid spray on their hand and face causing 2nd deg burns to face.

While removing a socket liner from a raw mill using a hammer and pin to knock a bolt out, the EE that was helping was hit with a metal fragment and it entered EE's eye. The employee was wearing safety glasses at the time.

While aligning liner up against frame, the choker slipped down a little causing the liner to slip down smashing the tip of EE's left ring finger.

EMPLOYEE WAS PREPARING A MIX MATERIAL TO POUR ON THE LILN NOSE RING, THE MIXER LOCKED UP AND HAD TO DUMP MATERIAL OUT, THEY PLACED MATERIAL ON PALLET AND WAS USING A FORKLIFT TO MOVE THE MATERIAL THE PALLET WAS NOT LEVEL, EMPLOYEE PUT PRESSURE ON ONE SIDE OF PALLET AND LEFT FOOT WAS CAUGHT BETWEEN PALLET AND FORKLIFT RACK

Driver suffered a major health condition at cement load out, driver was transported to local hospital where driver was pronounced deceased by medical staff.

Process Attendant was completing inspections of the CKD Silo dust collector. As EE was climbing down the ladder EE felt and heard a pop in EE's right wrist. PA reported to EE's supervisor immediately. PA reported issues closing EE's pointer finger and gripping but no pain outside of that.

Employee was climbing a staircase in the finish mill building when they missed the last step and fell onto left knee. Employee was taken to a local clinic for further evaluation. Clinic provided employee a knee brace and EE was told to not climb stairs or kneel. Clinic also advised employee not to stand for prolonged periods of time and to rest knee every two (2) hours.

Maintenance mechanic was working on line 1 coal mill and was utilizing a come-a-long and other mechanical tools. Mechanic reported soreness in right elbow. Elbow pain continued to worsen until medical treatment was advised on 08/28/2020, at which time medical professional diagnosed lateral epicondylitis (tennis elbow. Employee was given tennis elbow brace by the doctor.

Employee was picking up an elevator bucket that weighed approximately 50 pounds. As they were removing the bucket out of a chute, they felt a sharp pain in lower back. Employee was taken for follow up and has x-rays scheduled.

Employee was descending a short, stationary ladder. As EE reached the ground, EE lost footing and fell. Employee used EE's arms / hands to brace the fall and injured the left wrist when EE hit the ground. Employee was taken for x-rays and they showed that EE broke the wrist.

Employee was attempting to unplug a chute. EE opened a port on the chute, and shortly after the chute became positive and blew dust into EE's face and eyes. Flushed eyes immediately at the facility and then transported employee to medical facility for further evaluation. Facility continued to flush eyes and had an ophthalmologist complete an evaluation.

An employee was descending a staircase in the shipping area when they felt a pop in their knee. Pain was severe so employee was taken for medical attention.

Employee was attempting to clean an area at line 2 cooler side door. While using an air lance the air hit a hard piece and forced dust back up into face. The material went around glasses into eyes. Flushed eyes continuously for 30 minutes. Followed up a few hours later and injury had not improved; so sent employee to receive medical evaluation.

A maintenance intern was observing mechanics replace pans on a conveyor when a co-worker pinched the intern in jest, startling the intern to the point where the intern's left knee buckled and the intern fell to the ground. The fall aggravated a previous knee injury to the point where medical attention was required.

A maintenance mechanic was utilizing a 4-foot ladder to remove siding from a hopper. Employee fell approximately 3 feet and felt pain in lower back. Employee was immediately taken to a nearby urgent care facility to be evaluated and was released on light duty.

Employee was on the 7th ring of the tank was to move the drive out ladder, employee was not wearing proper PPE and did not follow proper procedure in moving drive out ladder. Smashed RT Ring finger between ladder and panel.

Employees were installing last panel on the 7th ring of silo when one employee did not communicate properly with the other in moving panel. Employee could not get EE's hand out of the way in time before panel smashed it. This resulted in a pinch point of EE's left index finger. Open laceration to the tip of the finger and nail bed being torn off.

Employee was moving trail cable for dragline when ee felt a sharp pain in lower back. Employee was taken to urgent care for medical attention was diagnosed with lower lumbar strain was given anti-inflammatory prescription and a prescription for pain.

Employee was assisting a plant employee move forks on a lull when the forks were rocked catching the left middle finger tip of the K&G employee causing it to blow out.

Employee was holding up the front of the ABB analyzer reinserting it into L1 kiln another employee was running it in when the employee holding the front felt a sharp pain in employee's right arm, extent of injury is not know at this point was referred to Ortho for evaluation.

Employee was using a small sledge hammer to open the door latch on top of railcar when EE felt a sharp pain in EE's right elbow. Employee was sent to urgent care for evaluation employee was diagnosed with tendonitis.

Employee was Shoveling material spill on top of kiln 2 Preheater roof. Dust from the spill was being blown from slight breeze and dust got into employees eyes.

The MT was using a 3/4" pneumatic impact wrench to drive a 1-1/2" socket for removal of flange bolts when the socket shattered. The MT could not locate a rated socket so made the decision to use a socket designed for hand use, resulting in the failure. The employee received and small laceration to EE's upper lip requiring 2 stitches.

Employee was moving an electrical box and the box pinched the tip of his right middle fingers between the box and a fan housing.

IE was closing access port when the tip of his right middle finger was caught in a pinch point. IE sustained a fractured finger tip and laceration.

Employee went to generator room to take break and wait on crew EE leaned on table and came in contact with a Machete that was on table cutting his left ring finger.

On 10/01/2014 employee stated that he was feeling pain in his lower stomach and groin area while operating a Woma gun. Initial medical examination was negative for hernia, employee is continuing treatment and the final diagnosis is uncertain at this point. This injury report is being filed as a precautionary measure.

EE was reinstalling ABB analyser into the feed shelf inlet when the inlet became positive from cleaning that was taking place above blowing open port and hot kiln dust coming from port hitting employee in back and upper neck causing 1st and 2nd degree burns to the back of the neck area.

EE was cleaning build up out of the tower when a large chunk of build up fell on the end of the WOMA gun pulling EE forward into the thermal coupling that was protruding from wall striking him in the nose resulting in a fracture.

Employee broke the tip of his pinky finger when it was caught between the lid and housing of ABB analyzer while employee was trying to clean filters.

Employee was putting tipping valve weight back on tipping valve arm when shaft rotated to the up position causing weight to slide down shaft arm and pinch employees left ring finger between weight and valve body. (Employee was suspended for remainder of work schedule upon further investigation Employee was terminated for violation of Lock out/Tag out policy.)

EE was trying to close bottom hopper door on coal car, using a 20 ton hydraulic jack to close last few inches to pin in place. Bottle jack slipped, causing door to spring open and pinch EE finger between the door and grizzley grating.

EE left hand was caught between dump hopper and forklift mast after dump hopper was sat on ground EE attempted to push hopper of forks, hopper tilted backwards pinching EE palm and thumb.

EE and co-workers were removing the existing platform on a tire feed system. The platform was cut loose and held by a chain fall. EE was holding the hand rail or the platform when the platform moved pinching the ring finger on his right hand between the hand rail and a diagonal brace resulting in laceration and broken bone of his right ring finger at the first joint.

Employee was laying on floor of Kenworth truck connecting the throttle rod that had just been replaced. As employee was exiting the truck through the passenger side, he missed the lower step and fell backwards hitting the back of his head on the service truck that was parked approx. 3 feet away, resulting in a laceration.

EE was washing out buildup from the pre-heater tower calciner with the woma gun, when a large slab of buildup fell from above on the end of the woma gun, causing the gun to slam employees right hand little finger between the east calciner coal pipe and the woma gun, causing a laceration and fracture.

Employee was installing a lid on a new material tank, wind was blowing, material from job site blew into employee's eye.

EE and co-workers were unloading duct work off the boom truck. When the duct was lifted, it swung off the truck and pinned the EE between the duct and a golf cart parked in the area bruising EE's left leg.

EE alleged that he was lifting an 80 lb gas bottle when he felt a sharp pain in his lower back.

EE was working in the Raw mill with over head cutting taking place. EE went to put on a pair of gloves to remove some metal from the area that had been cut away from the job taking place over head. When he put on the right hand glove he felt something like he had been burnt by a hot ember. But it was a brown recluse spider in his glove that had bitten him.

EE was washing the calciner and a piece of buildup fell on the end of the lance that was being using raising the back of the lance up and striking the EE under the chin causing mouth to slam shut and jamming teeth together. Chipping his front top tooth.

EE alleges that he was lifting a D-ring on a 4-way chocker, when he felt pain in his groin area.

Employee was discarding a bag of garbage that contained a peace of metal siding. When he attempted to throw bag into the dumpster it reflected off the side of the dumpster and bounced back. The piece of metal that was in the bag stuck through, cutting the employee on left bicep.

Employee was stripping forms. He slipped and fell backwards and the form fell on his leg. He did not report injury till 3 days later when he was crawling on rebar. He complained to Supt. about some pain 6/14/07.

EE was walking beside rail road track, checking in rail cars that had come in the previous night. While walking on rail road gravel, EE twisted ankle, falling to the ground.

EE was lifting a 100,000 lb A-Frame with the 100 ton Link Belt crane using two 7,300 lb nylon chokers. one of the chokers slipped shifting the load to one choker which broke allowing top of the A-Frame to fall across the cab with the injured employee inside.

Employee's thumb was caught between forklift cage and handrail as he was attempting to hook cable choker around forklift to remove welding machine from upper deck of rail loadout.

Employee was removing dust buildup from around lower door of calciner. EE said it was a very windy day and when he was removing the material it blew back into his face and eyes. The dust high of lime caused redness to both eyes.

Employee was supervising calciner wall fabrication on 3rd floor of preheater tower, wind was blowing at high gust causing dust to blow into face.

EE was installing under table plates. The plate was bumped by another employee and fell in the mill through the beam opening. It struck EE knocking him down. EE got back to feet, came out of small door and was going down stairs. Another employee made him lay down to check injuries, rescue was called.

Employee was reinstalling cover plate on roller pair. He was holding plate with one hand and installing bolt with other when it rotated and struck him on the arm.

ROBOTRAC HAD BAGGED DOWN FROM BEING OVERLOADED WITH COAL. EE WAS USING AN AIR LANCE TO FREE MATERIAL. BELT SURGED PULLING AIR LANCE AWAY FROM HIM. EE LOST FOOTING LOSING HARD HAT. AIR LANCE HIT SIDE OF MACHINE THEN CAME BACK AND HIT EE ON TOP OF HEAD.

THIS ACCIDENT OCCURRED WHEN A LARGE PNEUMATIC WINCH BROKE FREE FROM ITS ANCHOR. THE WINCH PINNED THE INJURED EMPLOYEE AGAINST AN I-BEAM STRUCTURE. THE ACCIDENT CAUSED CRUSHING INJURIES TO HIS LOWER TORSO (CRACKED PELVIS, TORN BLADDER).

WHILE UNLOADING MILL SCALE AT RAIL UNLOADING, EE CLIMBED INTO CAR TO REMOVE MATERIAL THAT WAS BRIDGED ON THE WALLS OF THE RAIL CAR. WHILE ENTERING RAIL CAR EE STEPPED INTO HOLE IN MATERIAL CAUSING TO FALL FOWARD

While putting belts on the coal mill fan, employee was rolling the belts on when his glove got hung between the belt and sheave at which time the tip of his left middle finger was pinched causing him to snatch his hand out tearing the tip of his finger off above the fingernail.

EMPLOYEE WAS UNLOADING A RAIL CAR OF MATERIAL. HE HAD CLIMBED ON TOP OF RAIL CAR TO CHECK IF 3RD POD WAS EMPTY, AT SAME TIME RAIL CAR WAS OFF BALANCE AND STARTED TO ROLL. EMPLOYEE JUMPED OFF OF CAR LANDING ON GROUND INJURING BOTH WRISTS.

WHILE PERFORMING AN INSPECTION OF THE FEED SHELF LOOKING FOR MATERIAL BUILD UP ANOTHER EE SET OFF AN AIR CANNON WHICH BLEW HOT DUST OUT OF PORT HITTING EE IN THE FACE.

EE WAS HOLDING BULL PEN WHILE CO-WORKER WAS HITTING THE BULL PEN TRYING TO KNOCK OUT WEDGED BOLTS OUT OF TABLE LINER. CO-WORKER MISSED BULL PEN SMASHING HAND WITH SLEDGE HAMMER.

EE WAS TAKING A BUSTED BAG OF CEMENT OFF OF CONVEYOR WHEN HE DROPPED IT ON THE DECK, CEMENT POWDER BLEW UP IN HIS FACE, GETTING SOME MATERIAL IN HIS LEFT EYE.

PLACING PART OF LOADER THAT HE WAS DOING MAINTENANCE ON TO REMOVE BEARING. PART SLIPPED AS HE TRIED TO CATCH IT HIS FINGER (LEFT RING) WAS PINCHED BETWEEN THE PART AND PRESS.

EMPLOYEE WAS REPLACING A METAL PLATE AND HAD ONE END PROPPPED UP, SEVERAL INCHES, WHILE POSITIONING THE OTHER THE UNATTENDED END FELL AND HIS END PINNED HIS FINGER AGAINST ANOTHER METAL EDGE.

WALKING ON GRIZZLY OF COAL HOPPER, FLOOR LEVEL, AND HE SLIPPED AND FELL, TWISTING HIS ANKLE.

EE WAS STEPPING ONTO A LADDER, TO CLIMB DOWN, AND HIS FOOT SLIPPED OR CAUGHT ON TOP RUNG CAUSING HIM TO TWIST HIS BACK.

THE EE WAS INSPECTING A REGULATOR INSTALLATION WHEN A 2" HOSE/CLAMP CONNECT DAME LOOSE AND THE PIPING/REGULATOR STRUCK HIM IN THE FOREHEAD. THE SYSTEM WAS PRESSURIZED W/APPROX 80 PSI.

EE WAS SHOWING TWO OTHERS LOCATIONS WHERE BOLTS NEEDED TO BE REPLACED OR TIGHTENED WHEN THE DRAGCHAIN CAUGHT A TRACK GUIDE SECTION IT BROKE THE BOLTS THAT HELD IT IN PLACE AND THE SECTION SNAP PED OUT OF POSITION. THE METAL SECTION STRUCK HIS LOWER JAW/CHIN AREA.

EE WAS USING A HAND HELD GRINDER TO GRIND METAL FROM A RECLAIMER BUCKET. THE BUCKET WAS RESTING ON THE EDGE OF THE SHOP TABLE. IT FELL FROM THE TABLE & THE GRINDER STRUCK HIS RIGHT HAND.

EMPLOYEE WAS CARRYING EQUIPMENT ACROSS AN AREA OF WET CONCRETE AND KILN DUST ON THE CONCRETE CREATING A SLIPPERY CONDITION. HE SLIPPED AND HIS LEFT ELBOW STRUCK THE CONCRETE SURFACE, CAUSING T HE CUT TO HIS ELBOW.

EMPLOYEE WAS STANDING ON THE SECOND RUNG OF A ACCESS LADDER AS A HAMMER DRILL WAS PASSED DOWN TOHIM. HE WAS NOT READY TO RECEIVE THE DRILL & AS HE ATTEMPTED TO CATCH THE DRILL HE FELL TO THE FLOOR AND THE DRILL STRUCK HIS HAND AGAINST THE CONCRETE FLOOR.

EMPLOYEE WAS ATTEMPTING TO UNPLUG THE OUTLET OF STAGE ONE AT THE PREHEATER WITH AN AIR LANCE ANDHOT DUST BLEW OUT OF THE ACCESS PORT. HE WAS WEARING SHORT CUFF GLOVES & DUST BLEW UP THE SLEEVE OF THE HOT SUITE JACKET.

EE WAS AIR LANCING MATERIAL IN A FEED HOPPER & THE HOSE CONNECTION AT THE BALL VALVE, CAME APART THE LOOSE END OF THE AIR HOSE WHIPPED & STRUCK HIM ABOVE HIS RIGHT EYE CAUSING HIM TO FALL OFF THE HOPPER HE FELL APPROX 5 FT TO THE GROUNDWHEN HE FELL HE INJURED HIS LEFT WRIST/ARM

EE WAS CLIMBING OFF A RUBBER TIRED CRANE AND MISSED THE BOTTOM STEP. HE FELL APPROX. 30" AND HIS HEELS AND THEN FELL TO THE GROUND ON HIS BACK.

THE EE WAS LOOKING UP AT THE TOP OF A SILO AND STEPPED BACKWARDS OFF THE CONCRETE BASE OF THE SILO. HE FELL APPROX 18" - 2' TO GROUND LEVEL, LANDING ON HIS BACK / LEFT SIDE.
