employee was starting up the plant. couldn't get dryer to start up. They locked out the power to the dryer, and went to try and turn it manually. While turning it their thumb got caught and they got a cut.

3rd party truck driver got stuck picking up a load. Employee used loader to help get stuck truck out. While backing up - the loader made contact with the front of the truck. This caused the truck door to close and make contact with driver's knee. The driver does not have an MSHA ID.

Employee was at work for 4hrs and 20mins when they started feeling a sensation on their left hand and side. They tried to gather themselves and figure out what was happening. Coworker called ambulance to be safe. Paramedics evaluated employee and didn't find any cause. The emergency room didn't find the cause of the pain. Employee was sent home and given a few days to rest.

Employee was running dryer. Needed to grease bearing. Didn't shut down dryer. Went under guard & tried to grease while dryer was running. Fore arm was caught on something and EE's skin was removed.

Employee was working on sand spill due to malfunction. While trying to walk around the pile employee hit head on a catwalk. Employee was not wearing helmet at the time & suffered a cut on forehead.

Employee was using Lull forklift to cover bag load from rain damage. Boom should not have been used to perform task. Boom was all the way out while traveling, forklift started leaning. Trying to balance the forklift, lever got stuck & tipped on side. Employee will be retrained on using the proper equipment to perform the task.
