Employee was dusting blinds in the office. While trying to reach the highest point of the blinds employee jumped up, and twisted right ankle.

while replacing a water hose fitting to preform housekeeping duties. Employee cut EE's left index finger with a knife.

Employee came in to start shift. Was seen acting disoriented and 911 was called. Employee was taken to the hospital.

Employee was prying a piece of grating up when the pry bar slipped and hit EE above EE's right lip resulting in a small cut.

Employee climbed up short vertical ladder to look at belts on elevator. When descending ee stepped off ladder to platform wrong and twisted ankle.

Employee was inside the screen box replacing the shaft for the shaker, all equipment was locked out and tagged out. While putting the new shaft inside the shaker, Other ee assisting him could not hold the shaft and it fell approx. 6 to 8 inches and struck him on the left side of his head. It put a cut of about 2 inches on the left side of his head. He did not lose consciousness

EMPLOYEE WAS GUIDING AUTOMATIC SWEEPER FELT PAIN IN LEFT SHOULDER.

EMPLOYEE WAS STACKING BAGS WHEN HE FELT A PAIN IN HIS LEFT SHOULDER.

CLEANING OUT BOTTOM OF BAGGING MACHINE.

EMPLOYEE WAS DISMOUNTING THE FRONT END LOADER IN11-99 AND INJURED HIS LEFT SHOULDER BUT NO MEDICAL ATTENTION WAS SAUGHT AT THE TIME. RECENTLY HIS SHOULDER BEGAN TO BOTHER HIM AND MEDICAL ATTEN TION WAS SAUGHT AND SURGERY WAS DONE ON LEFT ROTATOR CUFF ON 5-17-00.
