Employee removed side port hole to remove rocks and cement rushed out of port hole and got into EE's right eye.

Employee was in the process of training the belt conveyor, using a pry bar for cleaning around bearing bolts, pry bar slipped between the belt and the tail pulley "belt was running" this caused the pry bar to push the employee's hand against the conveyor frame resulting in a cut to the right pinky finger, requiring 2 stitches.

Employee was helping another employee replace the cutting edge on a loader bucket. They were both lifting one end of the cutting edge to get it into place to attach it to the bucket. When they were lifting the cutting edge, it raised up until it hit the bucket then it stopped abruptly. The employee felt pain in EE's back at that moment.

Employee state that they were busting concrete bags and some of the bags had rocks in them and a finger got caught in between two rocks and it caused a laceration.

Employee went to investigate a dusting situation at the silo discharge. EE climbed the ladder to the upper platform and found spillage on the platform. Upon trying to descend the ladder EE lost EE's footing and fell to the lower level.

Employees were removing trees with an excavator and a sling. The sling was wrapped around the trees to pull them out with the excavator. When one of the trees was being pulled out, the top portion snapped off. An employee on the ground ran to avoid the falling debris. However, the falling debris brushed across their left shoulder and back, injuring the left shoulder.

An employee was assisting with the installation of a guard. The employee pushed against the guard to help get it into place. When EE did this, EE heard something pop in EE's right shoulder.

Two employees were installing a metal cover on top of a silo. While repositioning the cover, one of the employees pulled up on handle forcibly causing the handle on the opposite side to go down rapidly pinching the end of the employee's middle finger between the cover's handle and the silo roof. This caused a laceration to the finger and the loss of the fingernail.

A weathered rope located on an exterior catwalk, exposed to the elements, was being untangled by an employee. The rope began to fray and splinter into small pieces. The wind caught the rope fragments. One of the fragments went into the employee's right eye.

The security officer was inside vehicle at the entrance to the dock. Two people approached the officer's vehicle to get permission to board the vessel. The officer was observed stumbling and struggling as EE got out of EE's car. As EE attempted to get back in EE's car EE collapsed. 911 was called. The officer appeared to have a brief loss of consciousness and internal pain.

Employee was using a jackhammer to demo a concrete step. When the concrete broke free, the jackhammer bounced hitting the employee's left boot at the edge of the steel toe. The jackhammer bit slid past the end of the steel toe, pierced the boot, and hit the employee's foot causing a laceration. An x-ray later revealed two fractured toes.

A portable welding generator was operating outside of the Auxiliary Mill Control Room. The diesel exhaust fumes were drawn into the control room and accumulated. An employee in the control room was exposed to the fumes and carbon monoxide. After receiving fresh air, the employee complained of eye irritation and headache followed by nausea, vomiting, and diarrhea.

A contractor was sweeping the floor when EE stepped back on a loose floor board causing EE's leg to fall through the floor. The board was cut to run ducting for a new air conditioning, and the a/c contractor did not properly secure the loose board.

A sledge hammer was used to remove a sprocket segment from a bucket elevator. When the sledge hammer hit the sprocket, metal came off and hit an employee about 15-20 feet away in the abdomen. The employee was taken to the ER to receive an X-Ray to verify no metal remained in the laceration, a tetanus shot, and two stitches before being returned to work.

The valve on the hopper was stuck in the closed position. Employee open the port hole to remove the rocks that were blocking the path and secured the port hole. He proceeded to strike the hopper with a mallet and the port hole flung open, the material sprayed out getting behind his safety glasses into both of his eyes.

WHILE GRINDING, A SMALL SPEC OF METAL FELL INTO EMPLOYEE'S RIGHT EYE. AFTER SEEN BY A PHYSICIAN THE METAL WAS REMOVED. EMPLOYEE WAS GIVEN EYE DROPS AND RETURNED TO WORK THE NEXT DAY (HIS SHIFT ENDED AT 1530 WHICH EXCEEDED THE DR'S VISIT). HE WAS WEARING A FACE SHIELD AND SAFETY GLASSES AT THE TIME OF THIS EVENT.

Employee stand next to co-worker who was using a jackhammer to unplug feeder in 10B Slag Cement Silo when a lump of slag came loose in the silo causing jackhammer to slip out of the co-worker's hand and hit ee in the l. lower torso. Accident occurred on 9/4/07, He did not report incident or seek medical treatment until 9/7/07.

Employee was installing an air canon on the South Docksider North vessel with witness. A piece of pipe with flanges on both ends was threaded to the discharge valve of the air canon. When he pulled on the 40 lb pipe it came out of the threads and fell 4 ft onto his right foot. The pipe landed right where the end of the steel cap ends on his shoe and crushed his big toe.

While repairing and cleaning inside of 9A Ramsey Feeder employee stopped the feeder to examine it, thought the feeder had stopped turning then put his right hand into the paddle wheel of feeder, but it was still coasting to a stop which resulted in severe cuts to all fingers on employee's right hand.

AS HE WAS STEPPING DOWN ONTO THE DECK OF THE SHIP FROM THE GANGWAY HE STEPPED ON A U-BOLT THAT WAS WELDED ON THE DECK OF THE SHIP, RESULTING IN INJURED LOSING HIS BALANCE, USING HIS HANDS TO BREAK HIS FALL THUS SPRAINING HIS LEFT HAND.
